If you have the option to pay with credit card always do, they have a good currency exchange rate. Whereas the 711 atm add an additional exchange fee so it's more expensive in comparison. But for the times you can only pay in cash i agree it's the best option.

It's almost certainly more expensive, I am a ex-flight attendant and this is the advice given to us by the airlines. Even with crew discounts the worst rates will be airports...followed by tourist spots like Shibuya and Shinjuku :(

Needless flying is terrible for the environment
For 1, it depends on where you come from. If you're from a developed country the overhead costs may be high so exchanging money at home will be more expensive. If you have a debit card that absorbs foreign transaction fees and foreign atm withdrawal fees, then withdrawing money from Japan's ATM would be the best choice. They usually only charge 200 yen. If that card you're using is Mastercard, then withdraw from 7/11's ATM during regular office hours because the 200 yen withdrawal fee is waived. As a rule of thumb remember to withdraw in local currency (i.e. JPY) if the machine asks so you don't get charged the ridiculous DCC fees. However, if you don't have that sort of debit card, then just go for a money changer.

There are atms that cater for this use case at the airports or you can draw at 7-11.
It's generally true for 99% of the time. for the 1% it would be some flash deals from currency exchangers who has excess yen OR the other exception is if you convert via a neo-bank card like Wise/Revolut/Travellex and take it out at a 7-11 ATM (Seven Bank) which will charge 110/220 yen per withdrawal
