The Oregon POLST Program has served as The Founding POLST Program since the early 1990s, creating what is now called Oregon POLST: Portable Orders for Life-Sustaining Treatment®. The Program’s ultimate goal is to carefully elicit, accurately record and reliably honor patient preferences to have or to limit medical treatments across all settings of care.

Oregon POLST 2023 Form for Health Care Professionals
In under 4 minutes, this educational video for health care professionals explains the essential changes and requirements in the revised 2023 Oregon POLST (Portable Orders for Life-Sustaining Treatment) form. Topics include: a clarification of new requirements in Section C to document who participated in the discussion to complete a POLST, how to locate additional resources on the back of the form, a recommendation to use an advance directive instead of a POLST for healthy older adults, remote patient signing in ePOLST systems, and more.
