    2:52 _"The remaining epithelium we will scrape off using a very blunt hockeystick."_ Thank god I got ICLs instead. 20/10 vision with no damage to the central cornea whatsoever.

    Excellent presentation, Am doing refractice surgery sice MEL 60, now i have MEL 90 with CRS Master. I will ask my all juniors to go this presentation.Trans Epithelial PTK is suggested for irregularly irregular Cornea, Can we do TE PTK for all four situations you presented ? I agree when commerce enters , science dies,

    What annoys me is that PRK is mentioned the first minute and after that there is only spoken about Smile and Lasik. So I still do not know how to compare Smile vs tPrk

      PRK damaged your eyes?? How so- what are your symptoms? I have been prescribed central corneal regularisation (a laser treatment) by my opthamologist. I would really like to know what your experience was.

    I am a refractive Optometrist working in two refractive centers in Pakistan as I am trained person and I know the pros and cons of refractive surgery if you people want to make a refractive specialty than it must contain Ophthalmologist and Optometrist who has the skill set and have same rights and give best to the community as Mother Teresa said ''You can do what I can't do. I can do what you can't do. Together we can do great things". Sir the reason why I want to address this cooperation because Sir I am the person who has seen all your lectures on UA-cam and read articles about it and teach to Ophthalmologist and you are the great mentor in this area of eye or Ophthalmology

    I guess it all mental. I would also love to get this operation, but i am afraid of all touching. Even if i dont feel anything, i would still be in fear, just because i odnt know what to expect... What advice or option cuold you recommend for very sensitive people?

      It’s the suction lens or whatever it’s called. It makes the eye steady.

    In an unoperated and/or in a retreated myopic eye, what amount of correction could be effected using the femtosecond laser to perform totally-intrastromal PRK? If CO2 and H2O are instantaneously created when corneal tissue is lased by the femtosecond laser, could a near totally-intrastromal PRK be made more possible if a small side cut were included to port/vent the gas and fluids instead of relying on absorption to clear the cornea?

    What about using MMC regularly for every PRK case? Also, what is your cutoff thickness for LASIK? For thinner corneas do you offer PRK for your patients?

      Yes, my cornea was on the thin side and I had 2 separate doctors recommend PRK.

    Mine my TransPRK Only Right Eye Surgery is August 10, 2022 after 1 week The Bondage contact Lens removed and my vision is blurry after a day It goes Double Vision in a Week today is August 27 My vision is better no double Vision i can see but not as clearly I Mean "Sharp", my left eye which is good And Sharp but so far so good Maybe a Month my eye Is near perfect vision or maybe perfectly complete... i forgot sometimes to Put an Artificial tears, but artificial tears must be used regularly after The TransPRK Surgery and Removing bondage contact lens for 2weeks and use mostly artificial tears "Lubricants" Like Dextran, Hyabak depends on what ARTIFICIAL TEARS That the DOCTORS RECOMMEND... 1-3months Or 1-6 months Recovery for the Epithelial Cells of the cornea when it is fully recovered your vision is Clear... almost (90-97)% my vision is perfect within 2 weeks..

    Share this video with new patients and reduce or eliminate pre-surgery Xanax. Is there any work on developing totally intrastromal femtosecond laser procedures for touch-ups or even for the [de novo] correction of myopia without lenticule extraction?

      @@markymark9957 It may be fairly common practice for clinics to give a pill to calm the nerves of LASIK surgery patients. Dr. Reinstein is so smooth and masterly in his chairside manner and performing the surgery, no xanax needed: just screen this video. What's up with totally intrastromal PRK femtosecond laser procedure research? Why can't the Visumax 800 do touch-ups or even eliminate the mechanical, surgical removal of the lenticule in favor of ablating all of the tissue without even cutting the epithelium?

      @@cylon5741 u answered real quick man. i.m not a doctor yet but i bet i could answer your question. if i understood correctly, u mean making the lenticule just go poof and disappear right? well. theres a problem with that. gas bubbles would be trapped in there, a lot of them. keeping this aside, it is impossible to destroy a lenticule entirely, without further damage to the eye.

      @@markymark9957 In other words, it's only an engineering problem. How confident are you that the molecularly unzipped corneal tissue would occupy more space than the mechanically removed tissue? If there were pressure problems, ceate very tiny vent ports, or have multiple runs under the femtosecond laser? FYI, try to look up IS-PRK, if you are interested. That is short for [totally] intrastromal photorefractive keratectomy. Early work was been done on porcine and possibly unsighted human eyes, if memory serves.

    I have -1.5 on both eyes. I went to a doctor they recommended ICL for me. I was extremely skeptical because I have very good insurance so money isn't an issue and they could do ICL at that place while they would have to drive me to a different location for laser surgery. Then I went to another place that basically told me ICL doesn't make sense for me because my eyes are too good they then measured everything and said that LASIK isn't an option for me and that they would do a PRK on me (the old alcohol one). Now I also made a third appointment with a clinic that also offers SMILE to get a third opinion because at this point I'm very confused. It is hard as a patient to navigate and understand the different motives that doctors have. This talk definitely helped me a lot, although as a non medical person I couldn't follow everything but this talk is off course not focused on layman's.

      @@bilalameer57 Smile. And I'm very happy with it. It's been over a year since. One of the best decisions I've ever taken. I have friends that did Lasik and PRK tough and they are also very happy with it. I wouldn't go for ICLs unless you really have a high defect.

    I did manage to reach a conclusion, so the femto lasik has less risk than tprk or prk in terms more protection against sun light? So i must wear sunglasses with prk and tprk and not with femto lasik?

    This almost frightened me from a surface type surgery... wearing sunglasses all the time out? The whole purpose of this surgery is to take off glasses completely Can't transprk laser do epithelial ablation on a point to point basis so that variable epithelial profiles is taken into account? Could you compare transprk to epi-lasik? epi-lasik does not use alcohol, so i'm feeling better compared to lasek/prk

    Generally, for someone with thin corneas (440 microns) what is more suitable PRK or LASEK? Recovery time is not a concern, long term accurate visual acuity is. Thanks

      Seem to work much better than any usual products. Minimum 1 day faster recovery, less pain and better film. PRP is still rather hard to get in most places and their benefit is debated - but they are at the very least as good as serum.

    Prof. Reinstein- Appreciate you sharing your knowledge on this topic. Given the amount of research you have done on the Epithelium, I’d be extremely interested to hear your opinion of Bowman’s Layer (function and/or consequences of losing it) as I had TransPRK done and am now missing a large portion of it!
