THE INJECTION TECHNIQUE
Hi dear colleague,
In this video I would like to show you a case from A to Z which I treated using the Injection Technique. This patient has ankylosis of multiple teeth and in order to get occlusion we decided to restore the teeth with composite.

    Extreme opacity is difficult to get rid of. Drill had to be used

      The reason the drill was used is that there was still some sound enamel covering the defect and even after many etching cycles I still wasn't able to penetrate this defect. In those cases you sometimes need to remove a tiny bit of healthy sound enamel to get access to the defect underneath.

    When do you feel that sandblasting is necessary during ICON infiltration? Most clinicians don't seem to do it right? Would it improve the results you think?

      I think sandblasting is one of the most important steps during ICON Infiltration. When we think about the infiltration our only goal is to fully penetrate / infiltrate the defect. Therefore we need to make sure that our ICON dry and ICON Infiltrant can reach every part of the white spot. If not, a part of the white spot will remain (the part that isn't reached). By using air abrasion we remove a superficial layer of enamel, which makes it easier to etch and penetrate the enamel (it usually saves us 1-2 etching cycles. If, after 3 cycles of etching we don't see anything happening, it's important to use sandblasting again. Because the etch simply won't do enough, probably because the defect is pretty deep. By using sandblasting we remove a bit more to be able to penetrate the defect.

    Hello Maarten, how do you remove these off patient’s teeth? Can patients wear these home for a few days and how would they carry on with OH? Will these lock-in to the teeth and be hard to remove?

      Hi, thank you for your questions. I usually leave it in place for 2-4 weeks (I use spot-etch and sometimes spot-bond to get a better adhesion to the tooth). Most of the interdental spaces are blocked, to get some more retention. So I usually advise to only brush for these 2-4 weeks. Once the mock-up is removed they can pick up their regular routine.

      Dat is afhankelijk van de casus. Voor esthetische mock-ups is soms 10 minuten al genoeg. De patient kan beoordelen of het is wat hij/zij wil. Voor slijtage casuïstiek of casussen waarbij je de occlusie/beet veranderd hou ik meestal een periode van 2-4 weken aan. In sommige gevallen langer, in sommige gevallen korter.

      You can use Powerpoint and email the photos to yourself, but nowadays there are also a lot of good iPhone apps that you can use to make lay-outs and posts.

      Haha I get that. If it's not in the bar you can use the your mouse and right click on the bar. You can adjust the buttons that are there and you can add the 'crop' button.

      I usually do to ensure I get a really nice polish. After checking and maybe adjusting the occlusion I only go back to the spots I altered. It's always easier to get a nice polish under rubberdam, than without.

      Thanks! Honestly, not sure! It's only been around for that long, so we don't know the real long term effects. But for now the 10 year results are really promising!

    Do you rub each tooth for 3 minutes each before washing off ? So in this case rub 18 minutes (6 teeth) per cycle in total ?

      Hi, 3min for the entire cycle. So I place it on all teeth, then start rubbing it in constantly switching teeth and then wash it off.

      Yes, it can be done, but I usually don't. ICON is most of the times an esthetic treatment and not many people will have complaints about a white spot on the occlusal surface of their molars.

    Does it really make sense to use the ICON if you still use a bur in the end to remove some of the anterior tooth surface to later cover with composite? You could've received close to equal result by just going directly to the bur and an aesthetic composite after initial whitening, which removed staining. Seems like a lot of unnecessary work and costs for the patient for similar result with this ICON, just my take on it but please educate me. Nice work on the video!

      Yes it does, since you are not removing all of the hypo mineralized enamel. It's still really minimal invasive. I used the bur to remove a thin part of sound enamel that was covering a part of the lesion which I couldn't reach with etching alone. Have you ever done ICON yourself? You often see that after the treatment you are left with a small dip in the enamel due to the fact that often these lesions have less enamel and you are removing a bit with etching. To get a really nice and shiny surface that doesn't show any dips you often have to cover it with a really thin layer of enamel composite.       What do tou mean didnt you see the teeth already where prept? Or sre you blind he only lowered the margint infragingivaly corrected margins and lowered the incisal edge 1 mm not sure what you would have done but it would have sucked

      Thanks for your response! Yes indeed, there were already crowns on these teeth. I merely removed them and adjusted the outline and lowered the incisal (metal) part a bit to give the technician some more space to cover the metal part of the tooth.

    Amazing video! My Sunday afternoon well spent watching all your videos :) I have a question, temporizing was done with a silicon template of the original situation or free handed? I think it's great that your patient has impeccable hygiene because a lot of similar cases present sometimes where the gums are less than ideal and bleeding is hard to stop with retraction cords. Cementing with flowable is new to me and I'm assuming you used etch and primer bonding etc...did you etch the restorations as well? And removing the excess flowable, it doesn't leave any open margins? Thank you for sharing this next level off the top quality video's! Helemaal geweldig xx

      Thanks!! Yes, the provisionals were made with a putty index that I made before I started my preparation. For me, patients always have to go to the hygienist first. I’m not doing restorative treatments in mouths with achter periodontal problems and calculus. Yes, I cemented with heated composite, there is an entire protocol that you need to follow to prepare the tooth and ceramic restoration.       Hi. MIH is “Molar Incisor Hypomineralization”. If you google it you will get a lot of resources that can perfectly explain what it is. Cross-Polarization is taking a photo with a special filter that filters out all of the reflections of the flash. Therefore you get a better understanding of the tooth, color and characteristics.

      Only a little bit to cover the defects that were present at the white spots. Most of the times you are left with a small depression, so I used some composite to regain the normal shape

      Thanks, het is eigenlijk gewoon een schuifmaatje om te meten of de elementen even breed zijn. Yes, ik wil graag dat ik in de diepte geen luchtbellen heb en dat alles goed aansluit. Ik doe dus eigenlijk een soort injection moulding / sneeuwploeg techniek hier. Fijne feestdagen!

      Thank you! I only use keynote to crop or rotate. I never do any post editing. I try to get my settings so that I don’t need any post editing. Saves me a lot of time.

      I leave them in the camera and have a spare set that’s always charged. Since i’m making so many photographs I’m charging my batteries multiple times a week

      I feel that the focus bouncers from Minesh are giving a softer light and that can sometimes look a bit more artificial. But they are a beautiful product and still on my wishlist.
