Doctor Explains Metabolic Associated Fatty Liver Disease And Why It's Important #zepbound #ozempic
Welcome everybody, my name is Dr. Brian Hartman. I’m an Emergency Medicine physician who is dual-boarded in Obesity Medicine. I used to work with Emerge as a telehealth physician, but as addressed in prior videos, we’ve closed that business and no longer provide direct medical care.
Emerge is now a concept a brand for health and wellness videos.
Today I want to talk about Fatty Liver Disease. Or Non-alcoholic steatohepatosis. Or non-alcoholic fatty liver disease. Or metabolic associated fatty-liver disease as it’s now called. This is a condition where excess lipids are deposited in the liver, causing an inflammatory state that can eventually lead to cirrhosis and liver failure. In fact, it’s the most common form of liver failure in the United States today. It’s passed hepatitis C and alcoholism.
We see this noted on abdominal imaging routinely. Incidental note made of fatty infiltration of the liver. It’s tossed to the side and usually ignored. But it’s a HUGE problem. It needs to be addressed.
I compare it to someone coming in for shortness of breath and we get an X-ray only to see a 5 centimeter lung cancer in their chest. Going back, we find a small nodule a few years ago on the X-ray that was mentioned but never followed up. Imagine your stomach starts to swell, your eyes turn yellow, and you get a crazy rash on your body. You go to the doctor and find out you have cirrhosis, and it was caused by something noted on imaging from years ago yet you were never told about it. Imagine you had elevated liver tests for years but no one said anything or bothered to treat you for it, and now you have liver failure.
I saw a patient with fatty liver commented on the CT scan. Their liver function tests were elevated and I discussed this with her. She said she already knew about that and her doctor was “watching it”. Not aggressively helping her with weight loss to stop this disease progression. Literally doing nothing other than repeating labs and imaging every year. This patient had heard about Ozempic for weight loss but never really looked into it because her doctor wasn’t concerned.
There’s no excuse for this in today’s medical system. If you have an ultrasound, CT, or MRI of your abdomen that comments on “fatty infiltration of liver” or something similar, it’s time to be the squeaky wheel. Inflammation of the liver is bad and should be treated, not ignored.
If you can get brand-name Ozempic, Wegovy, Mounjaro, or Zepbound, great. If not, consider compounded meds to get the same effect. You don’t want liver failure!
As a final point, it’s up to you to take control of your health. You can’t expect a doctor or anyone else to fix your problems, or care as much as you do about them. Educate yourself through videos like these and online groups, and then show up to your appointment with specific questions.
Doctors should respect you for caring enough about your health to be prepared and motivated to make a change. Now let’s get healthy, together.

I was on it for two weeks I was miserable my insides hurt so bad it kept me awake all night I stopped and feeling almost normal again

I finsh my last 2.5 zepbound and i got a back pain
Almost 2 months on zepbound. No weight loss. Queasy, hard to eat enough protein. Worst: don't feel like doing anything. Everything's kind of gray. It's called anhedonia, and it's not talked about much. Might be a dealbreaker for me.
Insurance companies are so short sighted. They will pay for a $200,000 surgery rather than a $1,000 per month medication.
No side effects so far. Not hungry, but 100% have to make sure I am meeting my protein goal. I also noticed I don't feel like smoking or eating sugary foods.
It's easy, Lilly is a money hungry pharmaceutical company and our medical system is a Fn scam.
Has anyone here been approved by TRICARE Prime?

I started Zepbound. But I started off with slow digestion before the medication and I find that I am hungrier on the medication. Is this normal? I just finished my first month dosage at 2.5 and my doctor is going to move me to 5.0 to see if that helps.
Should I wait until the wekend to try it? Meaning should i not do it during the week because i may have side affects at work etc.
That is exactly what I do! I take mine Friday evenings. Honestly, I probably was more scared than I needed to be and sometimes taking it on Fridays can mess up my weekend when most of the good food is around :). You should do whatever feels right for you, but its smart thinking to do it on the weekend.
So helpful! Wondering when I get to my goal weight will my insurance cover mantience? Prob not. Still grateful.

honestly surprised that you recommend cardio over resistance training. that is very outdated, especially for those on glp-1! look up the research for yourself folks! cardio is great for heart health and high blood pressure but sucks for weight loss!

I have to be transparent. I will have to cut what I’m putting in my 401 and cut my health insurance just to put my money into HSA to now pay out of pocket for tirzepatide. It’s to expensive and lifesaving

My endocrinologist has me taking 15mg of Mounjaro, )aka( Tirzepatide. I realize that this is the same formula, ingredients and medication as, Zepbound. But, isn't Zepbound dosed higher for weight loss? I thought I saw a video last night right here on UA-cam, saying it is. Thank you!

I appreciate you making these videos both for the education and the stigma that occurs with being overweight as it is such a complex issue. I’m a therapist and would like to also offer feedback re the comments on breastfeeding. If it’s research that suggests this can lead to obesity just say that. Instead the implication is mothers who bottle feed are forcing the full amount on their babies promoting a “clean your plate mentality”. I don’t think this is always true. I couldn’t breast feed due to medication interactions. But I didn’t prop my kid’s bottle up to drain into their mouth and leave them in a corner while I did other things. It was an interactive experience where I watched hunger cues such as pulling away, etc. I was breastfed and I’m overweight and take Zepbound. My adult children were bottle fed and are both skinny. Thankfully they didn’t grow up in a home that led to complex PTSD and depression, which according to Medscape, leads to sleep disturbance and binge eating. I know it was a small part of your presentation but society makes women feel defective because they bottle feed, when like obesity, it’s a complex issue and doesn’t mean the parent automatically becomes a formula pusher.
What do you suggest to increase energy level while taking Zepbound? Thank you

Sir we can only buy this if your rich..I'm not.bcbs don't give a flip that I'm in serious trouble if I don't get the weight off..u know God is watching them n how they letting people die
I am curious if you have any information on combining doses to achieve a higher dose. Example, injecting 2x2.5 to get to 5mg or a 2.5 and 5 to get to 7.5mg. I don;t think it acts the same. Was curious if you had thoughts on it. Since the shortage, I see many people doing this. I had to do 2.5x2 after one month on 5 since 5w as not available.
The cost is not something I can sustain for more than two months
I just got prescribed this and am afraid to start because of the side affects.
The side effects are overblown. I have had zero side effects. I take the shot every Saturday.

@ Yes! So excited! since august I’ve lost 27lbs. I love this drug! Unfortunately my insurance doesn’t cover it so I’m paying out of pocket with the coupon. That’s still 553.00 a month and I really can’t afford it any longer.

Did you start taking it and were there any side affects? Took me a month and going back and forth with the the Doctors office, pharmacy and insurance to finally get it paid for. Total nightmare but excited to finally be on it. Only trying to lose 50 pounds. Spent my whole life thin but once I hit 50 has been impossible to keep weight off.
Diarrhea bad every week for 2 to 3 days after shot

I started Zepbound several months ago and I needed an ICD-10 of obesity, not diabetes (I don’t have it) in order to use the company discount. My insurance plan covers no GLP1s.

I just watched all five of your videos. I started this adventure 12/20/24 at 302 lbs. I am now 245. I'm a 56 year old 6'3" male. I've been using Zepbound and it has been a fantastic solution for me. Since taking it my A1C has dropped from 6.1 to under 5.6. Wish I had seen your videos before starting because the guidance is excellent and would have helped me in some areas. I am on the 10.0 mg now. I've been on that dose for a month and opted to go for another month on the 10.0 mg before stepping up to the 12.5 mg. I will likely level off there. At least that's my plan. I've been up around 300 lbs for so long, friends hardly recognize me. It's been great! Best of luck to everyone and thank you again for the great videos.

Tip just be you. If I wer a Doctor I would just speak from my heart, no paper, no i pad, computer, nothing i would just talk to people from my heart and tell them exactly what it is they must do to loose the weight from taking medications, ETC. Because when you are speaking to others and being honest and showing compassion as well and understanding,you have a better chance of getting others attention. Because honestly i like to speak or get advice from A Doctor who is really being and showing compassion understanding, and humility.
I started Zepbound 3 days ago. I literally have NO appetite at all. I know this will “jerk” the weight off. What I’m worried about though. Is if I fast like this. Will it make me sick?

My biggest symptom from Zepbound has been lack of energy.
Has this improved? I had my first dose yesterday and feel like crap. Fatigue insomnia and headache
I just paid $1124 for my first month of Zepbound. My insurance does not cover weight loss drugs. I am budgeting for this medication for at least 6 months .
Why don’t you take the compounded version? It’s a quarter of the price.

Oh man. First day on Zepbound 2.5mg. Last night at 4am, I woke up in a sweat and with my heart pounding at about 150bpm. This lasted a couple of hours. Not only was it very uncomfortable, it was quite scary too. It felt like a full blown panic attack. I got up, vomited once and checked my heart rate to make sure it wasn't too crazy high, because it felt horrible. It's now 10am, but my heart rate is still about 103 after lying in bed for 4 hours. I think I'm ok now, but I certainly hope it doesn't happen again. It was awful.
