Tip for Hospice Leaders: considering patient acuity when determining caseloads helps your budget
Tips for Hospice Leaders: considering patient acuity when determining caseloads is crucial to ensuring all the patient’s needs are met.
Acuity is simply determining how much time each patient requires and providing each nurse enough time to meet all the patient and caregiver needs.
In hospice we have 3 areas of Acuity to consider:
1. Physical Acuity: This refers to the patient's physical condition and needs (wounds, drains, ostomies, etc).
2. Geographical Acuity: how long it takes to get to the patient (distance, terrain, traffic, parking, etc).
3. Emotional Acuity: This focuses on the patient and caregiver’s emotional well-being, coping mechanisms, and support system, etc.
By taking into account these different types of acuity when setting caseloads, hospice agencies can actually save money in several ways:
1. Prevents burnout and expensive turnover in staff.
2. Decreases after-hour calls.
3. Increases patient satisfaction and quality scores.
4. Decreases live discharges.

    Can you do a segment on good tips for when the patient is actively dying, dies during the visit, or has just died ago. I was hoping to learn from experience what works best but I do understand each family is different. Just wanted for the death day. I am not referring to medical interventions but referring to what emotionally can help the family the most? I am hoping to learn good tips to emotionally help the family on that day.     When my friend was dying of cancer, they sent a truly terrible nurse who never should have been allowed to practice without direct supervision. He would vamp all the time because he was in over his head and would not admit when he didn't know something. There was a meeting once about Annie's respiratory care and the guy told her something completely off the wall. The other nurse with him was shocked, immediately intervened, gave the correct information, and MUST have spoken to him after leaving and/or reported him. Annie called hospice to request another nurse. However, the director got angry and told her they didn't switch nurses. We just didn't call him anymore for appointments if she didn't need anything specific. I felt very badly because, being a nurse myself, I had encouraged her to try hospice. What a terrible experience it was.

      I’m so sorry for your loss, and that you had a bad experience with the hospice nurse, and the hospice agency. Most hospice nurses are amazing, but like any profession some are not. I know it’s too late to help your friend now, and for that I’m truly sad, but so you know - you can switch to a different hospice anytime. There is a website called Hospice Compare that will list the hospices in your area and help you to find one that meets your needs. ❤️
