It should be standard practice to require inclusion of all drug contraindications in the cohort matching. If contraindicated you should not be in the study, because that results in a less healthy placebo cohort so obviously the drug cohort will have better results.

I think the confusion between retrospective cohort studies and case-control studies arises because with both approaches, at the point of setting up the study, cases (those with the disease) exist - right now. A retrospective cohort study is like jumping into a time machine and assembling the exposed and non-exposed groups and following them forwards to the present time (when some have become 'cases'). Since we don't have a time machine, historical records are used in RCSs. When assembling the two groups, it is of course critical that we don't know which ones went on to become the present-day cases.

Why is the SE calculated from 1 sample a good approximation for the spread of many means of hypothetical samples (samples that we don't take in reality?)
