Yes, the bandleader-keyboardist in the video and the gentleman in the still photo are one and the same person. In English, he spells his name "Ofer Callaf". Some linguistics: In Hebrew, there is no indefinite article ("a" &"an" in English); there is only a definite article ("the" in English). Therefore, intuitively, I would translate the song's name as "A Prayer" instead of "Prayer". One text configuration that you say "keeps popping up" gives the credits - just as any music video would.
