Reading about PRP can be very deceptive. Your explanation was very straightforward. My Orthopedic Doctor went over all options, but he also cautioned about the cost of PRP. I decided I would just be putting off the inevitable and chose TKR. Thank you for sharing. Take care!

Had total right knee replacement in 2001 and a right knee revision 8 months later..had m.u.a. done after both surgeries with intense physical therapy. Was able to work the next 19 years with a very limited range of motion and severe pain. Best pain control for me was a tens unit while i worked and bad days all the time and also gabapentin high dose. I had right total knee revision 2020 that was very very successful. I currently 4 years out from surgery walk 4 miles a day. My problem was my body treated the knee replacment as foreign object and scared it over. I had tuberculosis when i was a child and still have scare/ calcium covered tuberculosis in my lungs at 68 years old. being older the last replacment has not scared over and walking with a normal gait to pain ratio. Occasional tylenol or cheap arthritis cream for tight muscle band around knee cap. Some people body will just scar up no matter what is done. My recommendation is to avoid knee replacment at all cost. Pain is not a reason to get it replaced period. For you can get it replaced and have a poor quality of life afterwards. Only see ortho specialities recent out or residency so they are up to date with current new procedures. Yes older surgeon has more experience but not in the advancement of knowledge new surgeon has today.

Wow thank you. Have those 10 questions in mind. TKR coming up in few months. Arthritis plus knock knee. I want whichever surgery is best for fixing this darn knock knee. Your videos are my go to for info.
I was doing well after my second TKR. A physical therapy student messed up my knee. I was in constant pain. I had to stop therapy. Because of that I am 8 weeks out and can barely bend my knee. I have resumed therapy. I am really angry. I have probably missed the window of opportunity.

Thank you for this review and I think it will help our patients. As a joint replacement specialist, who does both approaches I have found the literature supports the fact that there is more symptomatic extra bone formation, iliopsoas bursitis, tendonitis, greater troch bursitis/gluteus tendonitis and of course 40% of patients will have lateral femoral cutaneous nerve compromise after surgery with 11% that do not resolve by 5 years after surgery, all of which are much lower or avoided completely with a tissue sparing mini posterior approach. As such, after 25 years of orthopedic surgical practice, I have gone back to mini-posterior approach.

Lots and lots of physical therapy, icing and pain management
I had bilateral total knee replacement 10 weeks ago. I'm at 100-105 bend. Doing PT 3x a week. I wish my surgeon had been as frank with me. Thank you for passing on your knowledge

Hi Dr. Rosen, Thanks for a very informative video. I am scheduled for robotic assisted TKR in four weeks. My biggest concern is increased risk of complications from osteoporosis caused by long term prednisone use (Crohn's disease). Specifically from the use of the tracker rods/pins. I learned from one of your other videos that there is an increased risk of fracture or infection . I assume that risk would be higher for my situation. What would be your typical approach to a situation like this ? Again, thank you for all of the valuable information on all of your videos. I hope you are doing well.
The fracture is a risk, but low. If there is severe osteoporosis talk with your doc about your concerns.

I have osteoarthritis in every joint not only my knee.
Doctor is knee replacement possible for the age of 33? I had a distal femur fracture last year and my knee isn't bending. What should I do?
There is no age cut-off. However, some insurance companies attempt to deny knee replacements in the very young. At that age you would most likely need a revision or two in your lifetime depending on how long you live and how active you are.
Some people are happy others are suffering so it seems this operation is a lottery.
Yes, but the odds can be stacked your favor. #1 find a good surgeon. #2 do the therapy. #3 is biology which you can't always predict. I see at lot of second opinions for bad outcomes and 9 out of 10 times its due to #2

Thanks for the detailed explanation. How long has this been around? I had both knees replaced in 2023 and have no idea if they were done with robotic assistance or not. Maybe not. Anyway I am very happy with the outcome of my two surgeries.

Wait, I don't "need" a knee replacement? Despite having had my leg turned backwards at the knee, tearing OUT 3 ligaments and fx the tibia? RIght. YOU deal with it.
Ok I was back and forth on doing the surgery Scheduled for the 8th Nov.I changed the date . Finally after this video I’ll do it. I’m looking forward for your videos on robotic tkr. My knee is knocked. And everything else is pretty bad. I was leaning towards Manuel PT. That wouldn’t change the knocked knee . That is my deciding factor. Thank you

Isn't it true Doctor that the femur neck breaks first before a person falls?
We believe that happens some times listening to what the patient describes. It is usually when there is severe osteoporosis.
I wish I'd seen this before my operation. It should be made mandatory before knee operations. I never saw a physio for 5 weeks apart from the one quick session of a short stairs session of 5 minutes, then released home !!

My knee replacement was 6/19/24. I can bend it to 123, however if I kick back it’s like the knee kicks out, rotates and makes a loud sound. The result is pain. There is also a pain behind the knee 9 times out of ten. Can you help?
Hi Adam, I had my left hip replacement for osteoarthritisback in 2017 when I was 53. I recovered really well without any leg length discrepancy. I was aware my right hip would need to be replaced in time and I had my right hip replacement on October 1Oth. My surgeon said my right leg is now 4mm longer than my left but he says this is negligible. It feels very real to me right now. I know I might still limp for a while post surgery but when I do my exercises, my right foot drags along the floor and I am so worried about it going forward. I would be so grateful for your help.

I’m 80, moderate osteoarthritis both knees but not much real pain, only stiffness. Problem is over last 2 years right knee has become knock knee. This is causing balance issues and limping and I’m concerned it will begin affecting back and hips. Is knock knee with non-painful arthritis still a good reason to have knee replacement? My life is very much affected by this knock knee.
Yes, some people get bow-legged (there arthritis is worse on the inside of the knee) or people like you that are knock-kneed (arthritis worse on the outside of the knee) in either case if you are having pain from the arthritis and it is affecting your quality of life or ability to walk a knee replacement may be a good option
