    I have ben providing these services for my family and my in-laws for years for free. Great concept and probably something people could start savings accounts for if they don't have medical family members to cover these services when needed. These services are life saving and a much better investment than a kitchen remodel. So much backlash for the fees, but nobody judges how much people spend on eating out and drinking alcohol. I am considering doing this on the days I don't have clinic. Great video!

      Yes! It is unfortunate that something like this is needed, and it's too bad that our healthcare is so expensive, but that doesn't mean a patient advocate shouldn't be paid their worth. You are paying for their expertise, and especially if they are a provider, that expertise took many years and LOTS of money for them to acquire. You are also correct about priorities. People put all kinds of priorities above their health. Again, I'm not dismissing how expensive healthcare is, I'm just agreeing that patient advocates should not receive the backlash. They can really help people, as well as sometimes actually being able to help people save money on healthcare. Thanks for watching and thanks for your excellent comment!

    I can speak from a tele-neurological standpoint, PAs and NPs are not preferred over each other. Sorry for the mistype.

      By "other", I assume you mean MD. So, you're assertion, I'm presuming is that an MD is always better than a PA or NP, correct? In family practice, I would definitely argue against that point. There are many excellent PAs & NPs, just like their are many excellent MDs. If a patient has a good relationship with a PA or NP, than they may easily be the preferred provider. In specialties, yes, most often the MD would be preferred. However, I would probably want to see a PA who has worked in neurology for 20 years over a new grad MD.

Sorry if I got defensive, we are so often put down in the medical system that I just assumed it was another attack. 😜 So I’m curious, if you don’t mind me asking, from your experience, is there no difference because your organization doesn’t care what the laws are, or is it that you just practice in a state(s) where are the PA and NP laws are pretty much the same?

    Thank you for the information. Started test in 2024. I have reviewed my questions and Have 5 that have been replaced from the 1st quarter and 1 from the second quarter. That being said, my score did not shift at all. Is that updated after the second quarter is finished or is the score I received after I finished the second quarter (about a month ago) my score. I am passing but would like a little extra cushion. Do you know when scores are updated with replaced/removed items? Thank you.

    Donald Trump is the Catalyst for what started the opioid denying of people who medically needed the medication not getting it because a medical condition demanded it because of medical tests that verify they needed medication because of a health condition most doctors will not write your prescription and Medicaid and Medicare may not cover it because if you're in most States as I'm in the south it goes through a bunch of legislative stuff controlled by politicians and the company will deny the medication these are alive sustaining medications like anything else as blood pressure medication or insulin because you needed to stay alive most doctors are afraid of losing their medical license that's why they go through a song and dance of a documentation and a piss test every time you come into the goddamn office I'm God damn sure set the elites that are not getting Medical Care through Medicare Medicaid don't have to go through what these people on these medical policies go through to get their pain medicine Donald Trump has somebody get his pain medicine if he needs it I just have to get on his knees to some doctor godmother fucker to get his goddamn medication people need to see what a devil this guy is and his wake he's left a lot of people still in pain and people have died from lack of medication either through suicide or natural death because they could not get the medication I pray to God that the people who are denying decent American citizens medicine it would be great if one of these high and mighty doctors or politicians would have to wind up a Medicaid or Medicare and have to get on their knees for pain medicine

    I'm not a medical professional of any type however, I've always been deeply interested in all of the medical disciplines including ARNPS,PA and specialist PAs and ARNPs . I've been care for by both LPNs,Rn's and arnps of all skill levels and levels of sonority.

    In your opinion, what is the best DMS program out there currently?

      Honestly, I don't have a lot of first hand knowledge about the different programs, so It would be hard for me to answer that. What I can tell you is that most all of the programs offer different tracks within the doctorate. So, your options narrow down when you identify what you want your doctorate in. Meaning, it could be something like enhanced clinical medicine knowledge, leadership, public health, or education. Once you have decided what you actually want to get out the degree, then I would start identifying the programs that will give you what you want. Once you have that narrowed list, then I would look at cost, time commitment, and specific curriculum. I bet at that point, you would only have a few programs to consider. Then you can ask around about those particular programs.

    I feel like these questions aren’t concerned with real world scenarios. Many questions are testing our medical “trivia”. The one drug side effect, the one complication we all learned but never really see. Yes it’s important to be wary of but is it a way of determining if you are capable of practice?

      I get what you're saying, but isn't that true of any tests in medical education? We know in the real world that tests and treatments ordered are often different based on each patient's particular presentation, health history, and needs. However, how could you possibly test thousands of medical providers on those scenarios? I think it has to be generic or "medical trivia" in order to be applied universally. BTW - I'm not sure if you are aware, but the questions are written by practicing PAs who volunteer, so if you have a knack for writing questions and want to improve the test, you could contact NCCPA to volunteer!

      I don’t think that it will lower paramedics scope. First of all I don’t think that PA‘s/NP‘s will work in EMS significant numbers, so paramedics will definitely still be in high demand. Plus what PAs/NOs do is different than the paramedics. I think that the paramedics will pretty much still do what they do and then, a provider may be called in if it’s the type of situation that could benefit from being further treated in the field, avoiding a trip to the ER. as far as lowering the pay, I certainly hope not! However, I’m sufficiently jaded by the business of medicine, so this isn’t beyond the realm of possibility. 🙁

    I’m having a similar experience. One month into a NP job with Hospitalist. I’ve just been told that I’m slow and my knowledge is lacking. I spent 10 years as an ICU nurse and I’m feeling like my experience and education did not prepare me for this. Everyone says it’s awful in the beginning, but this felt worse than that. Only one month in to a 4 month orientation. It felt really unreasonable. “If you want to revel in insecurity then work in Medicine” how true.

      I’m sorry to hear that you are also struggling. It really is humbling and frustrating, isn’t it? You say one month into a four month orientation. Are you guys on the traditional 7 on/7 off schedule? If so, you really only had less than three weeks! Did they give you any suggestions or help to improve? I can recommend a good online course if you are interested. Just let me know.

    Thank you! I currently have the AAPA HIPPO program. It seems good for studying, but not easy for test taking- specifically, the SEARCH function is not user friendly. And, you cannot bring up one subject, i.e. Infectious Disease and have that as the sole subject to search. You have to search the entire program (i.e. drug through all the Blueprint subjects, so if you search the word fever and you're answering an ID question, you get drug through all the modules (e.g. CVS, RHEUM, etc) . In smarty PANCE, can you isolate search in each module alone, and not be drug through the entire database?

      Yes! SmartyPance recently launched that feature. You have the option to search across the entire entire blueprint, or you can search through just a blueprint category.

      It depends on state law. In Texas, you don’t have to be a medical provider to do it. However, it can be very dangerous if you don’t understand the facial anatomy.

    DO NOT WORK AT THE VA!! My wife is a provider at the VA and is being fired. I have a doctorate in business administration and wrote my dissertation on business ethics. I have rarely seen a less ethical organization. And I’m a veteran who received care from the VA. Just do yourself a favor and work elsewhere.

      Thanks! It really depends on how fast you are at charting. Some people document the minimum amount or have excellent templates and dot phrases preprogrammed and finish their charting before even leaving the patient room. I would say the majority work on charting during their lunch break, in between patients, and then finish as much as they can before leaving the office. Whatever they don't finish, they work on at home. I would say charting & paperwork is at least 50% of the job, if not more. Most primary care providers work on salary, so any charting done outside of regular business hours is basically unpaid. If you work for an hourly wage, it would be paid, but you would probably be questioned by administration and told that you need to figure out how to get the work done in the typical 40 hrs/ week. Medicine in the US is definitely a business now and they (administration) don't really care how much work or how stressful it is for providers. We are just supposed to produce whatever they deem is acceptable. Sorry to sound negative, but that is the reality of working in medicine in the US now days.

    I felt like I could relate to everything you said when I worked in the ED as a new PA. I quit 10 months later and although it's still something I think about here and there, I am more proud that I stuck it out for that long and had the courage to explore other things!

      Yes! We are often so hard on ourselves. Why make yourself miserable trying to force yourself to do something. Obviously, you need have some staying power and it's not good to give up too early, but if you know something isn't right, then change it! And I love what you said about having courage to explore other things!

    Thank you @TheMedicineCouch, another very helpful video. While it is re-assuring that not many lawsuits result in loss of licensure but it probably does affect future job prospects because its something you have to list in each and every future job application I imagine.

      You're welcome! Yes, I would think it would make job hunting more difficult, but I'm not sure the impact is as drastic as we think it would be. I could be wrong about that, but here's my thoughts. 1) Everyone in healthcare understands the "game" and I think realizes that anyone can be sued and they sometimes "lose" for many reasons. One of which is that the organization finds it cheaper to settle than fight. So, I don't think seeing a malpractice judgement on an application is necessarily an automatic decline. 2) I've seen multiple stories of physicians who have had their licenses revoked in one state, sometimes with multiple malpractice judgements against them, that go on to get licensed and hired in another state! Crazy! Having said this, I suspect it may be easier to overcome these problems as a physician. 🤷

    I got new 1 who decided ut was smart idea to take me off something that was working now put me on something that cause me now to not be able to see , puking every 30 min and shittin blood and due to my spinal condition i can even walk and ended up having to physically crawl get absolutetly 0 sleep and end up screaming in pain for 24/7 and to be gas lit

    Well, I applied again! Year #15 of trying to get in. I’m currently a GS 11 step 7 with a crappy agency. I was referred again for the VSR (Rater) and VSR position on March 23rd. Today it’s May 7th and my hopes are fading again. Why is this agency so difficult to get into?

      @@TheMedicineCouchI just had an interview today for a VSR position! I was hoping for a VSR (Rater) position but this might be the only guaranteed option for me for the time being. As a GS 11 step 7 with my current employer do you think they will try to match my current salary if they give me an offer? I know the VSR is capped at GS 10. Would they possibly offer me a 10? I haven’t gotten an answer yet from anyone who works for the VA.

    U.S. healthcare system is "problematic"....period! Hence the mass exodus of clinicians out of clinical positions. Until and unless there's a massive shift in mentality away from the RVUs, productivity, and profit-based performance metrics towards improved health outcomes, providers will continue to feel "miserable" and "stuck."

    I am taking quarter 2…I just was given a question that I had in quarter 1 that I got CORRECT in Quarter 1? How does that make any sense? I thought the repeated questions were to help you learn..?? Why not give me something I got wrong and see if I learned from my mistake? (And possibly improve my score)

    I have conflicting opinions about this. In a way, it sounds great. However, it seems like a small amount of ppl are who are benefiting. It seems a bit predatory. Our healthcare system is so incredibly confusing and broken. It is the patient who ultimately suffers, financially, emotionally. And doesn’t help the burnout issue with health care workers.

    Who pays for the services provided? The patient advocacy services? Is this a private pay business? How accessible is it to those most vulnerable? The concept is great, but it seems as though only those with the funds can receive this service. I’m halfway through the video, but so far this is the top question I have.

    The VA in Las Vegas is like a zoo. The hospital employees have no respect for the ill veterans. My spouse is sick all the time I am in the VA hospital a lot. The employees talk loud like they are at a party, they are very defensive when you ask a medical question. Everything in that hospital makes loud noise no respect what so ever. Some Doctors are rude as hell some Doctors honor their oath. It's a mess from A to Z I always ask the employees if they ever been in the military the answer is always no makes me wonder why they hire people who have no reason to " say' that you for your service'" and give service like they went to school to be ZOOKEEPERS.

    I’m honestly extremely dejected about medicine right now. Believe this could be a good route. My upward track as a PA has been halted due to medical issues, and most jobs that are hiring in my area are not offering fair market value. I just really don’t want to go back unfortunately. I want to be a PA but working conditions are just horrible. Any thoughts about PAs transitioning to research or computers? I thought about AI for clinical research but not sure how to start off.

      I'm sorry to hear that, but I completely understand. Many who work in medicine are feeling the same way, unfortunately. We do have lots of options, but I think most of us have a hard time seeing beyond clinical work or breaking out of the box we have put ourselves in.     Myofascial pain syndrome is the MOST COMMON manifestation of chronic pain. Myofascial pain syndrome is characterized by active myofascial trigger points, Myofascial trigger are taut bands in skeletal muscle or muscle knots. Continuing to use muscles with trigger points cause both peripheral and Central sensitization ie allodynia and hyperalgesia. Dry needling is the most effective way to treat active myofascial trigger points. There is always an underlying cause why the formation of myofascial trigger points continue. But let's really get into the science. Moseley, himself wrote the excellent chapter on pain in the equally excellent 4th edition of the text Clinical Sport Medicine(2012). Here are some Moseley excerpts: “Trigger points are present in all patients with chronic pain and are thought to reflect sensitization of nociceptive processing in the central nervous system… Patients with active trigger points present with persistent regional pain… These patterns clearly reflect central convergence of peripheral inputs, which further implicates the central nervous system in their generation and contribution to a pain state.” Moseley GL.     Why is this specific for PA-C? Can’t anyone with healthcare knowledge perform this task? Seems like an expensive degree to do non-diagnostic work that can be done by anyone who knows hospital policy and CMS. I bet the people who do this job are internally saying, “if only I had went to PA school like I wanted all those years ago….”

      It’s not only for PAs or NPs. You are correct, this is a job anyone can do that has knowledge of the healthcare system, as is stated in the video.       You could have ended your questions in a more professional manner without being ridiculous. She mentioned the importance of having a PA as your advocate- PAs provide more detailed explanations to the patient. They can see errors and go back to the provider, bringing information to the attention of the patient's provider. Help them save a lot of money by sharing different programs that could help them. Because they are not diagnosed, it does not mean that their degree is not being used. There are different ways for PAs to serve patients.

      @@zmoses6024 I wholeheartedly disagree with curbside advice. A healthcare audit means either nothing and requires zero medical knowledge or everything and will be held accountable in a court. I’m all about making friends and giving suggestions in a private setting, but when transactions take place, justifiable liability will always prevail. If just giving advice about health, go to a gym….lots of health gurus and quackery there.

      There is a certification available, but she clearly says that a certification is absolutely not needed. Most people don’t or understand what the certification even is at this point.
