Is cruxweaver addon ,but it seems not work for me TAT, it shows the icon but dont show proc and cooldown

@jacky1218000 can't offer you much support there, but there is a discord @ of the guy who created, he might offer you more assistance there. There was also a bug with the oder version that is outlined in the addon description on esoui

Yeah, being a weak dog that I can't cast ranged while boss is being pulled - there is no reason to loose off balance time to cast it

It is a mixture of codes combat alerts and our own addon, yes. Notification for knot pick-up and kite on 2nd boss are from our add-on

If you mean multiple boss health bars on the top: the addon is called "alternative boss bars". If you mean vertical boss HP of the left side of the screen: it the a setting in the addon "crutch alerts". Otherwise it is pretty standard LUI extended
