THEORY TIME!!! So, first off, I believe that only taking one bite wouldn’t normally cause such a violent transformation. However, according to your video, Nightmare has been bullied incessantly and all that negativity (such as anxiety, hatred, and panic) has formed itself into an alter ego of sorts. This "alter ego" has a physical form born from Nightmare's aura due to how strong it is. Nightmare is a sentient chrysalis for the goopy alter ego and all it takes is a tiny bit of outside magic to set it free. That's where the apple comes in. All it takes is one bite for that monstrosity to be set free. Did I get it right? :)
