This is a short & low quality video showing strand slippage during cable installation.
Grip failure can be happened anytime if the contact preparation is not good. It is not only wasting strand but also imposed crashing impact at deck level where the strand weight which is heavy drop at high level.
Always be alert at site and aware of surroundings.

Post tensioned rusty wires are an instant red flag. Having seen post tensioned wires before, they are meant to be completely clear of rust.
Hi, It is always the best to have a clean and clear strand surface. However, that's not always the case. Surface corrosion with no pitting still can be used. Once stressed, grouting will be conducted. =)

Recheck the design, add a few bars to compensate the loss of strength and deflection.
Yeah, but normally we will look at no cost impact by realign non structure element. =)
@ptinnovativedesignxdesignc1330 there is also a cost for that, time and manpower, but it depends.
why are your tendons full of rust
Light rust is still acceptable as long as no pitting on the strand surface.

What will be the bases of the permissible + and - deviation of theoretical and actual elongation?
Depending on which code that you use for PT design. We are using BS 8110 where under tensioning procedure, the difference is stated within 6%. ACI adopt 7%. Hope it clarifies.
