I used to love this game so much but it was a quarter thief! There was an old guy at the arcade with a bad head who used to hang around……he kinda looked like Karnov…. Sometimes he would give me quarters if I let him put his hand into my butt.
Was this old guy's name Herbert and did he try to lure into his basement for some popsicles by any chance?
Jumpscares... Hostile man pops up ready to open fire... Frankenstein Hatchet Zombie up close and personal! And why would the brains burst out of phial glass?

See my reply to @vgnid in the comments. There are some anomalies in this game, both on the MAME emulator and on the real PCB, especially on the last boss. It's bug in game.

Thank you very much, I love that game, I used to play it when I went to the arcade with my friends when I was a teenager. Thank you so much!
Maybe try older, or different MAME version. If MAME say about missing files, try to search romset with these files, and add to one rom zip. If you will have problem still, try to register on MARP retrogames forums, and ask 🙂

It is old version for replay. If you have problems go to MARP forum. I don't have any problem on newest MAME versions (I use wolfmame always).
