    Question for Dr Alex: For your patients who need to wear the fixed orthotic 24/7, how can those patients clean their teeth for teeth brushing and flossing? And how can those patients eat with the appliance in their mouth? Do you modify the appliance so that it is thinner and allows the patient to eat with it?

      Great question Tom! I give my patient a hydroflosser or a waterpik for cleaning. This fixed orthotic is ONLY in temporary, 2-3 months. It is not intended for long term. As for eating with it, the first few days can be challenging but, once adjusted according to the patients chew cycle and using DTR, eating becomes natural. Hope this helps. Dr. Alex

      @@UnclenchedPod Thank you very much for your reply Dr Alex. I definitely appreciate it, and that definitely makes sense. One last quick question: For chronic severe TMD cases with long-term disc displacement, how often on average do you and your ICCMO colleagues typically review the patients during phase 1 treatment? Is it once per week or fortnightly, or less frequently? My dentist in the UK seems to think that one appointment every 4 or 6 weeks is sufficient, but my muscles and mandible are changing every few days with my phase 1 orthotic which I started wearing 10 days ago. Thank you!

    Hello Dr. Alex and Dr. Priya. I live in Denver! Welcome to Denver and to the ICCMO conference. I'm currently being treated for TMJ pain by a Neuromuscular Dentist (since January 2024) and also an Atlas Orthogonal Chiropractor (since October 2024). I am so much better than I was a year ago - I was in so much pain and was desperate - but now I'm almost completely out of pain! And now I'm getting ready to make a decision about Phase 2 with the Neuromuscular Dentist that I see. I'm currently in a removable orthotic and he's recommending going to a fixed orthotic for a few months (to fine-tune things), and then to full mouth reconstruction. When I first started treatment, I thought after the original pain was gone that I could graduate to only wearing the orthotic at night - however he thinks if I do that my pain will come right back. We also discussed braces, however I would probably have to the wear them for a minimum of three years (I'm 66 - would rather NOT be wearing braces at my age.) He also thinks because of the complications with my case (upper cervical neck issues, 4 incisor extractions, several crowns) that it would be the best choice for me to do the reconstruction. But it's SO expensive ($50K out of pocket!) that I hesitate. That said, that's the direction that I'm leaning towards. My question is, #1: do you agree with his recommendation (hard for you to say without seeing me, I know!), and #2: do most of your patients that have reconstruction have good success with it? As in, does the pain ever come back? I hate to pay out all of that money and not have it work. What has been your experience? Thank you so much! I've listened to all of your podcasts - so helpful to me get through my TMJD journey!

    This sounds like ME!! Doctors look at me like they've never heard of weird symptoms like these. TMJ has just been mentioned. No pain , but constantly grinding!! and ears , eyes are involved.

    Hello!! Thank you for doing this!!! Ive had TMD since jumping off a cliff into running river where i fell face first into a rock that chipped my left front tooth. I had it crowned after a few years but my jaw has not been aligned and kept clicking for years.. sometimes migraines and pain. Found your channel via tiktok and now am here! Never TMI with TMJ!

    Hi nice to find you. I’ve been a TMJ patient on and off for the past 45+years. I’ve worn 6-8 various ‘splints’ over the years. The best was made by Todd Montgomery, DDS, DMD. Out of San Antonio texas Absolutely the most relief‼️‼️ I’ve also had 5 surgeries (1st 2 were one side than the other; the subsequent others both sides during the same surgery. By Jerrold S Goldberg, DDS, DMD. From Case western dental school director of maxillofacial department. I attribute my current level of next to no pain to the dentists mentioned above! In addition to my orthodontist from Park Ridge Illinois who’s name escapes me right now. I know it is not the norm to have surgery let alone 5 but i was in Agony ‼️‼️they tried injecting the joint, medicine, PT, restricting my diet, heat vs cold, manual exercise, and finally braces as an adult. I want also recalled by the government due to the false recording claims by Dow Corning of the joint replacements. Fortunately i am pretty stable at this point ‼️Ive never tried botox or acupuncture.

    I feel like I'm leaning forward and feel like I weigh 200 lbs. Flat shoes help or go barefoot. I'm 68, TMJ, for 8 years now. Everyday is a balance issue, but I hide it well.

    I have been following Dr.Priya for a while now and have actually travelled out of province seeking help for my TMD..I was told (by RMT)that my hips are out of line,my right side is shorter,forward head posture to name a few..crazy that you were discussing that today...So appreciate all you are doing to help people like me to cope with these debilitating condition..Will be tuning in for every sessio,Many thanks to both of you!
