Another sequel has arrived!
Something I've really enjoyed about getting the opportunity to review HYDRAGUN products when they launch, is seeing the updates that they make based on the feedback of their users. I reviewed the HYDRAGUN HeatPod version 1.0 back in 2022 and enjoyed it, but myself and others noticed some areas for improvement.
Well, that's where the updates come in, because with the HeatPod 2.0 HYDRAGUN addressed what were likely the main concerns they received about the first version. Namely, that most using these are always looking for hotter, and that the first version was a little short for the tall folks out there.
Version 2 of the HeatPod comes out swinging with an increased max temperature by 5 degrees celsius, increased length for the taller ones out there (myself very much not included there), with an updated controller to boot.

A Physio Named Jonah is a channel created by, well, exactly who you think. The channel features primarily Physiotherapy based topics, or things that it would make sense for a Physical Therapist to weigh in on. The goal is to make these often sterile topics interesting and easily accessible to anyone with a few laughs (or forced exhales from your nose if you're alone) along the way.
Jonah is a Canadian Physiotherapist who works with a primarily outpatient orthopaedic population at a clinic in Sudbury, Ontario. He completed his Master's of Physical Therapy at the University of Toronto, with his Bachelors of Sciences specialized in Human Kinetics coming from The University of Ottawa. He is also considered by his parents, wife and dogs to be a pretty cool guy.

    What is the point in making up nonsense names to differentiate between these movements? For example, there are two different leg press machines: 1) where we push on the platform and the platform moves (since the limbs go further away, this is considered “open chain movement”) and 2) where we push on the platform and the platform stays stable but our body moves (“closed chain”). These two movements are quite clear and obvious for people. But some crazy scientists thought of making things more complicated by adding nonsense names such as “open/closed chain”. And I ask: why? Why do we have to used these nonsense terms? It would be way easier to just define the movement as: the exercise in which we are stable and our limbs are moving or our limbs are stable and we are moving….

    Im also a chiropractor, licensed in 1996. I do GH joints (shoulder) adjustments seated AP with just a toggle. You are correct when you say that shoulder adjustments are not needed very often. Because of the mobility of the joint it’s rarely fixates. Most of the shoulder pain I treat is resolved with exercises and massaging the subscapularis muscle. I agree with the other chiros posting here though that this is not a shoulder adjustment, it’s a posterior rib. Rib adjustments are incredibly effective for relieving posterior rib pain, which can be excruciating. As the others posting here have already said, please stop commenting on things that are outside of your professional training. You are misleading people and making them distrust chiropractors and that will increase pain and suffering in the general population. Obviously Kirk Cousins was not harmed by this adjustment, and neither will anyone else be harmed who seeks treatment from a qualified and experienced chiropractor. Stick to criticizing PT or exercise videos, there’s plenty of misinformation out there for you to correct.

    What about the anti gravity concept. Like pull-ups. Doesn’t concentric contraction involve moving against a force such as resistance or gravity ? And how would it apply to a simple elbow extension and flexion. Thanks

    You said casually “it’s a business”, and I think that’s the root of the problem. You’re talking about only private for profit therapy as far as I can tell? About half of Canadian physios (including me, though i also worked in a clinic for 5 years) work in publicly funded hospitals. I don’t think you can address this issue without talking about the public-private tension. I think clinics lose physios because A) if people are primarily interested in taking care of patients with evidence based interventions, there are far better ways to do it and B) if people are primarily interested in making money, there are also far better ways to do it. Private physio exists in a weird incentive structure that often promote money over outcomes. Cheers! Kobir😎

      Good question! Based off of the orientation, what would be considered internal rotation (depending on your view point) gets named instead as upward rotation, or protraction. This is because of the vertical orientation/the fact that the scapula glides along the ribcage for movement!

      @AshR575 as far as I know there are no limits, but I wouldn't recommend working many more than 40 when caring for patients, burnout is a very real problem in the profession!

      Hey there! Pilates is definitely not my area of expertise but I'll give it a go :) A closed Kinetic chain mat exercise that comes to mind would be a glute bridge, whereas an open Kinetic Chain mat exercise could be the deadbug core exercise. Hope this is helpful!

    PT is great when you actually get to do it. There are day when you spend more time documenting than treating and since PDPM the groups and concurrent are crazy. It's insane to have 11 half sometimes 12 hours of treatment time packed into a 10 hour shift and then on top of that no company will pay overtime so you end up often working for free.

      Yeah.........yeah. Stories like this make me so sad to hear. All too common as clinics can prioritize patient volume and income over patient care quality and clinician wellbeing. Thanks for sharing, I'm hoping to hear that this is happening less commonly in the future, and help move towards a better care model for PT!

      Good point! The pec minor can definitely be a part if scapular depression. I often clinically implicate it more with rotation, but it has a role in both! The logistical reason I didn’t (to be very honest) was to keep all the muscles I included for scapular depression on the posterior aspect of the body. I felt it was easier to understand visually/conceptually that way. Thanks for checking out the channel!

      There are no BScPT programs in Canada anymore, so getting canadian licensing without a canadian degree involves going through the internationally trained application program!

      1. I went back and looked at that year, there were about $25000 in deductions from tax and employee contributions 2. Not sure which benefits you mean, but any structure of pay would likely be the same regardless of training location

      @ompatel1320 ahhh ok thanks for clarifying! So as an international student at a Canadian school, the same benefits and opportunities are provided. Once you attend a Canadian school, you're essentially treated as a Canadian student. Now if your PT schooling is from outside Canada, you will be unable to practice in Canada until passing through the Canadian licensing process. I get asked about this process a lot, so might be time to make a video on it once I figure out how to balance time as a parent more! Now, once licensed as a Canadian PT, all job opportunities are MEANT to be equal. I saw meant because unconscious bias and prejudice still exists, so will it always be that way in practice....sadly probably not. Hope this helps!

    Thanks for the excellent video. Two questions: how do the scapulae move in a lat spread and how much of the width of the upper part of the lat spread is due to the teres major vs the latissimus dorsi itself?

      Hey these are interesting questions. If I understand properly (please let me know if I’m missing it though) the lateral spread comes from the protraction motion when you roll your shoulders forward. That would be coming from Serratus Ant and Pec Minor predominantly. Teres Maj and Lats don’t actually move the scapula at all. Does that help answer your questions?

      Thanks for your answer. The scapulae do seem to protract in this movement exactly as you describe, but this raises a couple further questions. Re my original first question, scapular protraction seems to push the latissimus and teres major muscles out laterally, but do the lower parts of the scapulae also rotate outward in this movement? It seems like the visible width of the lat spread is due entirely to scapular motion pushing those muscles out laterally, not due to any contraction of the latissimus or teres muscles themselves; is this correct? Re my original second question, in weight training circles the common assumption seems to be that the visible width of the lat spread is due almost entirely to development of the latissimus dorsi alone, but when I look at anatomical illustrations, it seems like the teres major might contribute more to the visible width of the upper part of the spread. Is this right, or does the teres major contribute little to the appearance of the spread?

      Open. He remains stationary with the earth moving down as he pushes it with his arms. Experts correlate his workouts with most seismic natural disasters occurring over the past few decades.

    Why do we, physical therapists, receive a low salary number compared to dentists, nurses, and even the rest of the medical system, even though the number of years of study is six?

      Ahhhh a good question. There’s a few things at work here. I think important to remember in discussions like this is that salary is not dictated by years of school. There's a bit of a feeling in healthcare that it is because highly specialized providers often earn higher salaries, but this isn't always the case. Healthcare is a business at the end of the day (unfortunately). Salaries and the flow of money are determined by the value of service of goods provided (cost per visit or items sold), and the volume of how much is provided (# of patients). The main reason PTs earn less salary is our services are less valued (visits are cheaper either privately or allocated by hospital). Now, why this is the case could be quite a long discussion, so let's leave that for today. To try and combat this many PT clinics turn to increasing patient volume (more visits per day) but this generally leads to poorer quality care and higher rates of clinician burnout. All in all, PTs make lower salaries because their services earn less money when compared to other professionals. It's unfortunate and disappointing when you consider it takes 6 years of expensive schooling to get into the profession, but such is the state of affairs today.
