empyema is basically an exudate effusion right ?
Great question! Exudative effusion is based on the Light´s criteria and empyema is one cause of exudative effusion. However if you have exudative effusion it does not mean that it is empyema.
In diagnosing empyema which modality will be used to show the amount of fluid in chest and its exact location? CXR CT scans Ultrasound MRI
CT is often the modality of choice for further characterization of empyema, although other modalities may also be useful.
I think CT Abdominal Angio/ Tagged rbc scan/Capsule Endoscopy is a better choice for occult GI bleed
I agree that CT may not be the optimal modality of choice for occult GI-bleed.

What if CBF and CBV are decreased (such would be interpreted as large ischemic core) but except hyperdense middle cerebral artery sign there are no others signs of infarction on non-contrast CT ? Would patient in such a case benefit from reperfusion therapy?
Thanks, and a great question! In any case of reperfusion therapy, the onset and severity of symptoms plays a vital role as well as imaging. CT perfusion can aid in this decision, but I would be skeptical of the CT perfusion if it shows a large ischemic core but the NECT shows no ischemic changes since many errors can occur when processing the images for CTP. You could (if accessible) perform a MRI and look for diffusion/FLAIR mismatch to aid in this decision. I hope this answers your question!

Colon or large intestine ulcers can be seen in ct scan?
Gastrointestinal bleeds are hard to see with CT. Although they may occasionally be seen, a CT would not be the modality of choice.

This turned out to be biloma: an extra-biliary collection of bile, often iatrogenic and in this case likely related to the previous cholecystectomy.
