I relapsed after 5 yrs and 2 months clean and used the bernese method because I own my own landscape business and I can't take a day off. This was not anything like detox. It wasn't perfect but I recommend it as long as you are strong enough to know there comes a stopping point. I know doctors aren't a big fan but it works. Your video was so good.
Hi Doc. Thanks! This was my tool for finally getting clean. I started at night with .25 MG. In the morning, I took .25, and repeated at night. The next day I took .5 × 3. The day after, I took one MG ×3. The fourth day took 2MG × 2. The fifth day, I got off street drugs, took 2 MG × 2 (morning and night). The sixth day, I took 1MG × 2. I continued to go backwards in MGs, half off the day before till my last .25 Ten days later, I was off everything. My withdrawal was nothing. I'm now a couple of weeks off everything. My energy level is stable. I have a few days off everything. This tool worked for me.

It works! I am on day 2. I was nervous about taking the 2 mgs due to PWD in the past. I can feel the subs taking over and the DOC isn’t working much. I will up my dose of subs tomorrow no problem now that I know my body won’t go into PWD. Finally they found a great solution to this problem. Thank you Ken Starr! Your knowledge will save many lives and I am grateful for this video. If you are trying to switch to subs from DOC, this works. Just do the 7 day plan. You probably won’t make it to seven days still doing DOC because the subs will block it by then and you can just take more subs at that point.

@awfullysinned I see the original commenter didn't answer you, and I just successfully did this. I didn't have to stop using at all to start. If you can decrease the amount you're using that's great but it's not nessacary. You take .25 MG the first morning, and again that night. I took the same dose the second day, and then the third day I took .5 MG two times, day four 1 MG two times, day five 2 MG two times, day six 4mg two times, and finally on day seven I took a full 8 MG dose at once. At some point during induction you need to stop using, and really by day four you have enough of the medication in your system that you're not going to feel it if you use anyway. I tried the traditional way over and over and it didn't work for me because I couldn't make it through precipitated withdrawal. With this method I had no physical symptoms at all, I did deal with severe cravings between day 7-21, but my doctor increased me to 8 MG twice daily and it took care of that. I use a program called Bicycle Health to get my medication. It's all over the phone, so much more convenient than going to a clinic.

This works . White knuckle cold turkey makes the body relapse. Micro dosing or chipping tiny pieces will give your body time to adjust.

Hell I’m on 0.25 it wasn’t hard to get down to that but when I started I was on 2 for 2 years then went to 0.66 I had the 4 m strips I’m ready to coke of makes me numb but you right chipping is the way to go
Edit: I fully transitioned from 50 mg methadone/day to this method. It works completely. I will list only one small thing to be prepared for; even after fully transitioned, since you're going from a full agonist to an partial agonist, you can expect to feel (for maybe 1-2 weeks after) some hand tremor and mild anxiety. However, compared to having to be off methadone for up to 10 days prior to inducing on suboxone, this is WAY WAY WAY more preferable. Now, here is how I transitioned. If you are on methadone and want to know how to do this, here is how I started. Day 1 = 0.5 mg bup + 50 mg methadone, Day 2 = 0.5 mg x 2 (AM/PM) + 50 mg methadone, Day 3 = 1 mg x 2 + 50 mg methadone, Day 4 = 2 mg x 2 + methadone. Day 5 = 4 mg x 2 + 50 mg methadone, Day 6 = 8 mg x 2 + 50 mg methadone, Day 7 = 8 mg x 2 (NO methadone) and continue with no methadone. This was been a super easy transition for me. Only on day one did my legs jerk a few times, plus my vision was a bit blurry. However, overall I felt good. I have felt better and better and I believe this is going to be ok. Just wanted to share this in the event anyone who is on methadone and terrified. Don't be. This method is a game changer. Don't ever go back to the problem of having to be clean of methadone for 10 days. Use this method. You will be fine.

Since I could not really find anything like a conclusive plan I had to really search but even then all of the information was very general and not specific. So I hoped at least for others if it came from someone who went through this personally they might feel a little less scared to go through with it. I'm so glad I have my freedom back

Not looking forward to it.
@@Oakleaf700 Look, I've transitioned both ways. The way I described above is much much easier. Simply follow my protocol exactly as I followed the medical literature. I listened to a Dr lecture on this. He's done this 1000 times with never an issue. You'll be ok, believe it. You will see. God bless!

I began the transition last Weds 18th May 2021..I'd taken methadone on Tues, so began buprenorphine on Weds evening, cutting the first 2mg pill into 4. {the prescriber said to take 8 mgs in 24h}..I split it into tiny bits.
I could feel it ''rattle'' through my body, and with each new dose, my body 'rebelled'' a bit...3 days later, took 8mgs in one go, and it did make me feel really bad. Really sick, horrible headache, just wanted methadone back SO BADLY.
Stomach was paining bad and growling.
Later took another 8 mgs, and the pain produced by it was less.
Now take 8mgs twice a day {16mgs total}
HOWEVER
I am noticing my ''Fine motor co-ordination'' has gone awry.
Can't fill a kettle with a cup of water without spilling half, just so clumsy with my hands, when before It was really controlled. Definitely a bit ''Wobbly''.
But.....compared to having nothing, {eg, a stone cold methadone cease} it's much, much easier.
I wish I'd seen your info earlier.
I will screenshot it and give it to the drug prescribers. {specialists who work from doctor's surgeries}..wether they will use it or not, I don't know.

Today's my 8th day of this method of Suboxone induction, it works. I'm now taking 4 mg's 2 X's daily as of yesterday. Look, I've been struggling with OUD for 22 yrs, and I've experienced PW when attempting to start back on Suboxone, too many times to count, and I wish that on nobody. What I would add, is if you do try this, decrease your Opiate use slowly, and if it takes you more time to get to a certain dosage, then take your time.
Did you take your opiate and sub together or seperate
Not necessary to decrease your opioid of choice but increase in subs is required.
Since our minds want's to be High we tends to increase our dose more because the subs controls in getting further high.
But Funda is increase in subs is required for successful induction.

For anyone that is wondering, I haven't really looked in the chat but I did this myself recently and it worked perfectly. No withdrawals at all. A couple days where I felt lethargic but other than that everything was smooth.

I did a lot of research and exposed my sub doctor to this method. I'm currently doing it. Today I took 20mg of methadone and then just 3-4 hrs later went into the doctor's office. There they gave me lucymera just in case I went into withdrawal. After 20min I took .25 mg of suboxone. I didn't feel anything! Just the lucymera. Got me slightly drowsy. They literally printed the regimen from the online source I gave them. I'll be going up on subs and down on methadone. Completely painless. Feeling optimistic

I have been trying to get on buprenorphine for about a week and a half now I got super sick by taking them too early twice I live in Texas and due to the ice storm recently I was unable to get to the methadone clinic so now I have been doing this treatment that you have suggested and it is really working!! @@quincyhamlette6913 yes. Take you street drugs as normal. Then slowly start taking .25 mg of sub a day1. Slowly increase the amount of sub you are chipping over the days. Day .25. day 2 .25 twice day 3 .25 3 times. Days 4 1mg. Just keep on slowly increasing sub and eventually they will fully occupy your receptors and you won’t be able to get high anymore.
ive done this method on fentanyl and it works you just have to take an actual "micro" dose. 1-2mg isnt a microdose. like the doc said in this video about .25mg or 250 micrograms. when i did it i started off on just 125 micrograms and worked my way up over a week. Did something like this . day1- .125 ug + fentanyl day2- .250 ug =fentanyl day3- .250ug started cutting back on fentanyl day4 .5mg cut back more on the fentanyl day5- .750ug cut back a little more on the fentanyl day6 - 1mg still take fentanyl if you want day7 - 2mg bupe cut out fenanyl and i was good to go on whatever dose i needed of the bupe i needed to be good. i also was using 8mg subutex tablets. 54 411 to be exact . i was taking the little blue m30 tablets that had fentanyl in them which i had them lab tested and they had fentanyl and flurofentanyl in them.
Was on pain pills for 5 years then got on subs for 7 years. Ended up going to jail for a month after 2 weeks kicked the physical withdrawals of suboxone. Got released from jail and decided i was done with suboxone. Went 6 months clean but i noticed something was really off mentally not physically. Couldn't really figure out why i was soo depressed, lack of interest. After 6 months of bn misreable i stumbled and took a sub been back on them for 3 years now. Feel somewhat normal again. Truly feel trapped with suboxone. I hate it. I traded one evil for another evil
I hear u brother , I have been on methadone for 21 years ☹️, currently on small dose of 25mg (5ml) , every time I get off I go back on cause my mental is just not working ok, & I get scared it never will, but next time I try I’m going to take some steroids ( sustonon 250 ) , my thoughts are thinking that I will heal twice as fast (mental & physical) , I can’t find anything on this subject ( using steroids to help withdrawals) do u think it could work? ✌️🇦🇺
@@danielgonzalez-yp7hi try kratom to maybe give you some gumption AFTER withdrawals and then some more time. Hear it works great but do not take during withdrawal, it will make it far worse.

I'm 2 weeks [13 days] into a buprenorphine switch, and it isn't nearly as easy as I'd hoped.
Methadone was giving me heart issues..Long QT syndrome {only picked up by chance by a pre-op screening for a spine op} plus I wanted to give buprenorphine a go. Low dose methadone is not effective as you know, and does not ''Hold'' for 24h.
Buprenorphine sounded great..
But it's not an easy transition.
Leg muscle jolts, headaches and feeling sharpish pains and aches is not what I was expecting at two weeks in.
16mg bupe split into two 8mg doses daily.
Definitely have a clearer head, but the body?..urgh.
I think very long term methadone use is possibly the prob, but I don't know.
They said I could have methadone back, but I want to see how this goes...I like the clearer head with Bupe.

@@ghengiskhan6849 I had some hangups due to my insurance, but I am currently still microdosing bup with fent and was able to greatly cut down my use of the illicit stuff. Still a lot of work ahead of me, though. I'm just glad my Dr had heard of this and wanted to work with me on it. Dr's here in Maine are typically pretty behind the times when it comes to breakthroughs with recovery. But, no precipitated withdrawal since then, thank God. The dosing for microdosing has to be pretty exact though.

You can use intermittently on your first days of baby crumbs.. think of it as sneaking the subs into your body WITHOUT noticing. .25 of 8mg is tiny and I could use. Take .25 subs directly after and not notice at all- but that's what you want - to slowly get the subs into your receptors. And you start at .25mg or under bc its too low of a dose to trigger precipitated withdrawal. But if you do ever get into it- it's very dangerous you can override it you just have to have an opiate or go to the hospital
I will take at the most 2 mg of suboxone a day. My pain is gone and I can work efficiently effectively. I don't feel high I don't feel tired. I've been doing that forever ( i mean 2 years) and I'm prescribed 8 mg a day. It would be impossible for me to take that full amount. From what I'm understanding, this is the method that you are suggesting or that people attempt to eventually get too. For me, I'm not a physician I just know that that's worked well for me

Well I did it 2 times 2016, 2018 and after two times and so many relapses soon i have to go again the third time which is kinda nightmare stage, it is far away from easy , but let's say it is the easier way, and if you have clonidine which I don't.. for sleeping that is extra helpful.
so what do you do if you already built up the subs I take 4mg a day and stil get precip from the fent it is def way less harsh and way less intense but how do I seemlesly transition over to full subs do I double down on the subs or do I wait before going up to like 16mg or something
.5 milligram isn't a homeopathic dose. It's substantial enough to help. I was doing microdosing methadone in 2005. It worked
Does anyone know how well this method works when done to get off of Fentanyl? I’m going on 6 months clean myself(2nd time I’ve done treatment). The first time I went from BTH/ Vicodin to Subtext via the traditional dosage with zero issues. However, when I got off of Fet, I waited 3 days for first dose of Subutext(not Suboxone), & took 4mg’s as given in detox, and my precipts were horrible for about 7 days.
I’m trying to help advise a friend who’s interested in getting off, but is scared of getting sick, as he seems to have the same issue inducting. Would you take .05 of Bup(assuming Subutext works just as well if not better than Suboxone) the first day right when he wakes up PRIOR to him using the Fet? Or would he take the Fet first and then do the .05mg’s of Bup? I would think you’d want to do the Bup first. Please let me know if anyone knows this. This is a game changer if this truly works!
Bupe first. You still want to preferably wait as long as possible after using fent to begin the bupe. The bupe will still make u feel shitty so that's when you do the fent, as little as you can.
2mg is NOT a small dose for indiction!!! 0,5 or 0,25 is!
yes, I agree. Using the Bernese method with concurrent opiate use I would start at .25 to .5 mg but with traditional opiate induction I usually start at 2mg and it goes well.

Doctor using this method do you need to be clean off the fentanyl for a certain amount of time even if you’re going to micro dose induction 0.25mg a day??
The doctor said to continue your routines while microdosing. Try yo stop on the fifth, six, or seventh day (by that time, you should be up to two or three mg × 2. After a day or two off street drugs, go backwards in MGs, half off each succesive day. The process for me was ten days. I got off on the fifth day. The withdrawal was mild, more nuisance than sickness. Any lingering stomach problems, I took care of with Imodium. For anxiety I smoked weed. I've been off ALL opioids and opiates for one week. My nights are long, for I haven't slept in days, not even a nap. It takes a while for you to get that back to normal, as the central nervous system adapts, your brain shall adapt soon enough. Doc , so I’m doing almost about 2 gs of heroin (no fent in it I have testers) it’s not the greatest stuff so it’s not too strong the stuff I’m taking I got it specifically so I could cut down from using super good stuff at 3GS a day i was just trying to cut down . Do you have a timeline I could try for how much I should take of Subutex aka Bupe with no naloxone (that’s what I have .) like how much of a dose of bupe i should start Micro Dosing with and how long I should do so ?
trying this rn currently took .25 of suboxone after waiting 17/18 hours since last H & fent dose (DOC). while taking the subs I IV’d tar H & fent residue and felt what little was in that shot even after the subs dissolved. feel fine as of now, no precipitated w/d. Before I took the subs and shot I was in the middle of withdrawals and was in agony/pain as my receptors started getting active. It’s been fixed so far all is good, this is my last try and getting clean 🤞🏻

Ive done this method and it's definitely the way to go rather than stopping dope for days until you get full blown withdrawals then inducing..
Just to help give a visual that may help some "see" what the goal is here.
Picture 2 cups filled with liquids
One on the right is drug x filled to the top
On left the cup has just a small amount of Suboxone Suboxone (buprenorphine)
You gradually want to lower the opiates in the cup on the right
While at the same time SLOWLY start to gradually raise the Suboxone microdose (cup on left)
Eventually you want the cup on the right with the drugs to lower while the cup with the Suboxone gets fuller. Youll feel a little discomfort especially when the drug is lowered in your body(cup) and you start to have more buprenorphine getting into those receptors.... But likeothers have said this way is MUCH better than the other way of waiting days being sick and uncomfortable Again ust saying this to help people visualize. Hope it helped and i wish everyone success fighting your addiction. God knows ive been there myself and am lucky to be alive....
Ive had to do this method a handful of times but finally got it and have been clean for a couple years now. It may take 1,2,3 150 times but dont ever give up completely. I know this was from a long time ago. But go down .5mg every month. Until you reach the floor. I know some who have gone to .5mg and weren’t ready to come off and did .25mg the next month until off.
Hey I relapsed on two pressed blues. But it's been spread out over two or three days. But Thursday when I relapsed I took my Suboxone 6 mg in morning then few hours later relapsed so it's been 50 hrs since my last dose of Suboxone.Do you think the sub kicked the fake oxy off and didn't let it get to my receptors should I microdose and when should I start I don't want pw but I don't think I did enough plus it didn't feel very strong at all please help me brother thank you i dont wanna microdose with other opiates can i mivrodose with just my bup

I have a high paying job that ive only been st 5 months so im not covered by FMLA, i want to get on the sublocade shot so i can stop taking fentanyl, i recently tried ambulatory detox with klonidine and was unable to make it without using. I'm coming off fetynol day 2 I took a very small amount of Suboxone and didn't go into precip do I have to keep taking opiates for the. bernesse method to work or can I just keep taking small amount s of bupe?
If you can move your way up on Bup and not take any more Fent. That would be ideal. Give it a try

You can start taking .25 x2 a day
Then .5x 2 a day
Than 1MGX 2 a day
Than 2MG 2 A DAY
Then go backwards to 1MGX 2 a day.
At this point Stop taking DOC
Then do .5, then . 25 and then nothing. That’s how you do it and you will be good other than some anxiety and sleepless nights. Get some gabibentin or magnesium spray for restless legs and trazadone for sleep if you can get them from a doc
I took 3 doses of subutex aperently to soon after my last methadone dose and went into a violent precipitated withdrawl after the night I tried taking more sub every 8 hrs but still in withdrawl. What’s is happening?

I may be wrong but I think methadone Ur supposed to wait 24-48 hours ? Precipitated withdrawls are the fucking worst. I feel like some people can take subs soon after while others even if they wait go into PW. Another way is check out the withdrawal chart they use in clinical settings to see if u score high enough to take subs idk what it’s called but look up something like opiate withdrawal severity/scale/chart, rating or something like that.
idk this Burnese method seems like a good idea and makes sense in theory I really hope it works but I would personally be terrified to try it

But even then, it's tricky to get timing right. Even with running eyes and nose and shivery skin, and yawning, it's still a bit painful to take buprenorphine, unless a tiny fraction of a dose.
A friend {Who did not know of transfer} phoned, and said ';Why are you yawning, have you just woken up or something"?...So he could hear the yawns enough to mention them.

Did you successfully use this method to get off fentanyl. I'm down to 25 bags a day. It's about 1/2 gram. I don't feel a thing even if I do 10 bags. It's pure fent tho. I've just been functioning and religiously doing the 25 bags for 2 years now. I've kicked fent before and threw up bile for 3 weeks straight 24/7. That was a 50 bag a day habit tho. If your situation was very similar, please help!

Hello! I was hoping you could share with me any insight or suggestions about going higher up in mg during the microdose induction. I am stuck, due to fear of PWD, at 2mg. I am just so afraid to take more than 2mg at once. Also, I worry about PWD when stopping my full opiate(Fent) on day 7. Will there be any increase in uncomfortable symptoms when I start taking bigger mg doses and at the end when I jump off my full opiate and only dose the Bupe? **backstory if u need it...I've been on street Fentdope for 2yrs, 1g daily. Trying to use Bernese method to detox off it. I'm currently stuck at 1mg BID (2xday) for the past 3days due to above mentioned fears. I have been having slight WD symptoms after taking microdoses and I'm still using Fent. I have a family vacation planned next week (leaving in 5days) ... So I'm hoping to get on subs by time we leave for vacation. Any insight u have would be helpful.

@@dbcooper206 I honestly hate to tell you but I just had to face the Demon. Went through a horrible, precipitated withdrawal for about 48 hours. After that things leveled off. Now I am back to almost 100%. I take 8mg of subs per day. I’ll be cutting those dosages down to where I can get off of that as well. Thanks for your concern. Good luck to you.

I have the 2mg of Subutex and I have a pill cutter but if I cut it in half and then cut that piece in half it just crumbles, does anyone have any tips on how to get it as small as possible. I would really appreciate it.
I take sublingual I cut the 2 mg film in to 8 1/4 mg tabs. I"M down to 1/4 mg / day.
@@kewlmark72 she was wondering about the pills though lol. Basically Rachel what I would do is I would crush them up and weigh them on a tiny scale. That can weigh out very small amounts of things. Find out how much like .5 mg of sub crushed pill weighs and then go from there but you cannot cut them without them breaking up

Yes. I think that strategy will minimize acute withdrawal. You'll still be experiencing post-acute withdrawal, so be prepared for that.

My first time trying subs I went into some hell of withdrawals so I just got high again and I was back to normal. But the 2nd time I waited 48 hr instead of 24. And if work.

Replying to my own comment....6 days without Methadone...Buprenorphine taken as this video suggested really helped.
every initial 2mg tablet, I cut it into 4 x 0.5mg bits. It was feelable. .....I could feel it rattle through. but I am, so glad I didn't do the 2mgs at once. {dose was meant to be 8mgs for day one} 16mgs for day 2 and after,
A week later I'm on 16mgs buprenorphine, but still feeling a bit ''Withdrawally'' as in, pains in legs and jumpy as hell.
Methadone taken for decades is no joke to WD from, and I just hope the buprenoprhine kicks in eventually to make me feel better.

@@johnstewart1148 I am on day 20 of suboxone WD. About one year clean off heroin/fentanyl. IMO withdrawal is the price we pay for numbing our pain with opiates.

@@johnstewart1148 yeah my advice wait AT LEAST 3-4 days after last dose AND take niacin (it wont help withdrawal but will speed up it getting out of your body).
Then if you take your Sub.
if precipitated withdrawal occurs, just keep taking a half sheet every two hours until you feel better.

If a patient is taking a bundle a day, for 2 years, and does the Bernese method of detoxification.. will the patient be able to function at work during the detox?

Bernese method works, but what I found is I was still using gear without getting the high . Once your on your recommended dose you need to stop taking the street gear !

I’m old 70yrs. you tell me to go into Fentanyl withdrawal which is horrible, can’t sleep, dry heaving, stomach cramps, migraines, Restless leg etc and you ask me to take Suboxone so I can experience 10x the withdrawals as before?! I will kill myself if I don’t have a heart attack. Weeks ago I was on withdrawals so bad I was taken to ER and Dr said I had an irregular heart beat and could get a clot to the 🧠 brain
using the microdose taper method, the suboxone does not precipitate withdrawal. You gradually transition over.
Dr. Starr, if one is on Suboxone, and cheats and takes a high dose of Oxy* one time, must that person wait another 24 hours to take their next dose of Suboxone? Or would 12 hours be sufficient? Not expecting to feel much withdrawal as have been on the Suboxone for just shy of a week!

@@naomie3121 I'm sure you are wayyy past this. But just wait. Last time I was in rehab I laid in bed dying for 3 days from takin the sub too soon. And that time I waited 36 hours. The Dr. I talked to at the rehab said they are seeing patients need to wait 3 or more days to induce on a reg dose.

work with your suboxone provider to come up with the best strategy. There are a few options.

It's the buperenorphine that causes precips so yes you can do it with subtext or suboxone. The naloxone is not what causes precip

I didn’t know this exists and learned it myself like just from Knowledge of pharmacology I basically just did research and did the same thing without even knowing that it was a method and I am stable on 4 mg twice today and I’m going to begin slowly .5 maybe .25 weaning off I’ve also heard ketamine can remove all withdrawal symptoms
Ketamine definitely helps short term. Less long term, but worth doing
It helps me for the two hours it works but I feel just as bad afterward.

Scary experience...at the end of day 2 of beginning this Bernese Method, was jarred awake at 6am, in full on withdrawal... I followed this method letter for letter... Thankfully I took a shot and it went away... mostly... So this method isn't as smooth gping as it sounds... You definitely need to have your opiate ready to administer, because even the tiniest bit of suboxone can set off precipitated withdrawals.. I was worried that taking a shot wouldn't work, but I guess I have so little suboxone in my system that the fentanyl was able to get through... I don't know if I should continue this method... This might happen again and with a higher suboxone dose the other drug might not be able to get through.. Be careful with this stuff... it's no joke...
I hope all is well. I know you posted this 4 weeks ago. So I hope you're doing better. I did this recently and I know everybody is different. I did it with no issues at all. However, I wasn't using a lot. I was doing very little. But every morning that I would dose the suboxone I would always have to take a tiny bit of fentanyl or else I would feel the irritability. But as you're increasing the suboxone you decrease on your DOC. I'm sure it's not smooth sailing for everybody. But for me it really worked
