- In this video Dr. Angelish Kumar, a New York-based urology specialist, discusses the following:
● Although still in the early stages of research, preliminary evidence suggests antibiotic use can adversely affect the urinary microbiome, reducing protective bacteria and giving rise to pathogens.
● Further, it is possible that the bacteria not targeted by an antibiotic can develop antibiotic resistance after exposure during treatment.
● In terms of replenishing the urinary microbiome after antibiotic treatment, studies have shown that if you populate the vagina with good strains of lactobacillus, that they will also populate inside the bladder. Estrogen performs a similar replenishing function.
● Hiprex, a urinary antiseptic, works differently to antibiotics, preventing UTI-causing bacteria from replicating. It is only effective in urine which is acidic which is why patients may be required to take vitamin C alongside it.
● Though some advocate treating UTI with ibuprofen, Dr. Kumar explains that what is actually happening is that the ibuprofen is managing the symptoms only, while the patient’s immune system is left to clear the infection unaided. And, in some cases, this method could actually lead to deeper embedded infection because the body begins the repair phase before the eradication phase is complete.
● In some cases, an embedded UTI can be mistaken for a recurrent UTI. Tests can prove whether it’s the same set of bacteria causing each flare up of symptoms.

Dr. Kumar is a board certified Urologist who graduated with honors from The Tufts University School of Medicine and is a member of the prestigious Alpha Omega Alpha Honor Society. She completed her urology training at NYU Langone Medical Center in New York City. In 2017, she was the recipient of the gold star teaching award from the Albert Einstein College of Medicine. In 2020, she founded Women’s Urology New York, a unique practice focused on providing compassionate and comprehensive urologic care exclusively to women. In addition to being a diplomate of the American Board of Urology, Dr. Kumar is a member of the American Urologic Association, Society for Urodynamics and Female Urogenital Reconstruction, and International Urogynecologic Societies. She also serves on the Women’s Health Executive Committee for Springboard Enterprises.
