at the end showing the car accident, little misfortunes mother was drunk and hit little misfortune with her car. The accident happened when little misfortune was crossing the road and it happened exactly when the "underworld" glitch happened. Thats why Margo said "Remember, my game is the only thing keeping you alive."
The mom was in the house, the dad ran her over

The ending:
The "Little Misfortune" dimension is connected to the Fran Bow dimension. I suggest people to play it. Very fun. It's very bloody, a lot of mentions involving death, harm, mental issues, pills, and it's very haunting. There is a LOT of lore on that, if you want to know about the creatures or worlds. The ending to this game is supposed to be a "neatral/happy" ending, while the other was a "neutral/questionnable" ending. The game is supposed to leave you thinking. The reason the "true" (more happy) ending IS indeed "happier" is because the mother got what Little Misfortunue wanted all along. Even if she is dead, her mother decided to change for the better (as it appears). This is the best I can explain it without direct quotes of anyone. There are plenty of better, more in depth explanations to these games than this, but this is just an overall brief view.
I also really recommend people play Fran Bow. It’s on the longer side, but it has such an emotional story, and follows the inner struggles within the mind of Fran Bow that I can personally relate to on some levels.
A little side note I’d also like to mention is that the game is set in 1944. And the portrayal of Mental health “institutions” and their torturous methods of “help” at the time is hauntingly dark and depressing. A real morbid play through.
