@perioendo As one of our experience oral surgeon attendings noted: Extractions should be completed gracefully - not traumatically. Albeit, much easier said than done, especially when hooked under the something.
if you make the flap design like that when you suture wouldnt you pass the needle tip into the where the anatomical structures are that you are trying to preserve? (lingual nerve etc) especially if you want 3 mm bite for example.
when you do your distal trough are you removing a burs width of bone only on the distal buccal area. Or where do you stop to avoid lingual nerve problems?
sir,after giving incision and releasing flap and exposing the alveolar bone,is there any risk of uncontrolled bleeding that could be fatal during the procedure
dorjee bhutia d there will be no complication in giving incision..since there is no major artery present at that site..until and unless patient has bleeding disorder or undr medication.

instead of a distal trough couldnt you just take off the distal portion of the 48, and preserving the bone ?? like make a distal crown margin lol
