I had a fissure for over a year and half. Horrible. Colorectal doctor was great and I finally healed it completely. Here is what helped me. Pelvic floor physical therapy, breathing exercises, stress management, colace, Balneol, gentle fiber intake and PRANICURA. My fissure healed completely in 6 weeks after that combo started and has been gone for the last year. Unfortunately, last week, I noticed some minor bleeding and pain. Back to colorectal doc and he diagnosed a very tiny flare of my previous fissure (he said the scar tissue is agitated from a recent hard poop and he also said I had an internal swollen hemorrhoid but he wanted to treat it with conservative measures to not agitate my bottom. He’s very gentle.
I’ve already ordered more PRANICURA, and I’m doing stool softener (colace clear) and increased water and fiber (I eat bran cereal and I take one Metamucil capsule in the evening). And I’m going back to PFPT. I’m expecting to fully recover within 4 to 6 weeks, I will definitely continue with the panicura because it worked perfectly last time. And my doctor agrees with you, he almost never does LIS, only Botox in cases of severe spasm. Luckily, knock on wood, my spasming is not nearly as bad as the last time because this one is much much much smaller, and I have great faith that I will heal it up quickly.
Thank you so much for this channel. It’s incredible and we need more voice is like yours to talk about these often embarrassing things that I surprisingly large number of people suffer from.
One last thing. I have found that it helps to lube up before pooping with a tiny bit of Vaseline or AQUAPHOR. Makes things slide out and heals as well. I do AQUAPHOR before poop, and after shower, pat dry and apply PRANICURA. Amazing product. Everyone should at least try it! Also I’ve found that the least irritating poop for me is formed and soft but not too bulky. Fiber is great but I can’t overdo it, and colace clear is a miracle. And of course, PRANICURA!

My LIS worked for me. I don't know if it was soley responsible for my recovery or not. Massaging the internal muscle with a glove and lubricated finger was extremely helpful after I got over the initial awkwardness of it. Maybe it sounds weird, but somehow exploring in there and knowing exactly where the wound was and massaging the area helped everything relax down.

thank you for this! unfortunately I am in this exact position and I have been back on forth on LIS. I should say no one wants to do it and I don't want to unless I knew it would work and this is an interesting perspective.
I had a surgeons inject botox into my puborectalis and internal sphincter 100IU and it to be honest not only did I not feel anything relax over the whole three months, it mostly felt tighter! was harder to pass gas etc. having said that my pain levels were somewhat lower during the 3 months.
I am tempted to try again with the botox but why is it that no proctologists in the UK seem to be familiar with this because I now feel like maybe he did not inject enough or in the right places. I was under general anaesthesia at the time. WIsh I could afford to fly to you and get it done by you cause i know you would get it right.
