    How would you recommend documenting a verbal warning? Also, at what point does this document become part of an employee’s personnel file (if it even does) Is there a verbal warning or coaching before a ‘documented verbal warning’ occurs?

      Although it depends on your organization's policy, a simple memo or standardized format to document the reason for the reprimand and expectations for improved behavior or conduct should be placed in the personnel file.
