In Republic of Philippines all of the biological siblings have the same middle names legally
The Philipines before the American occupation followed the Spanish tradition of two family names (father's family name + mother's family name). The Americans changed that making the mother's family name into a middle name (mother's family name "middle name" + father's family name "last name").
Neither one of my parents had middle names but my sister and I do whereas our brother, who was named after our father, does not. I don't even use mine and the only few who know what it is are closely related to me. I just use the initial and some people get ticked off when they ask me what it stands for.
THEM: "What does your middle initial stand for?"
ME: "The first letter of my middle name now stop asking."

The reason why people have two last name is so you don't marry your blood cousin.
