  • It's Doc Tomson y'all!! 🙌🏽 I sit down with my doctor, Anastacia Tomson-Myburgh, who has been managing my medical transition for the last few years. We talk about progesterone and its role in HRT for trans women, and whether or not you should be on it.
    Most importantly,this video is not medical advice. You should definitely talk to your physician about what options may be best for you and your wellbeing.

      @@ZoeyBlack So, just some feedback....I added progesterone to my therapy in the beginning of March, and to be honest, I'm not impressed. It's really done very little to assist in my breast maturity and development. My doctor was not too keen on it either and only agreed because I begged her for it. Anyway, I guess it works for some and not for others.

    • Literal watery eyes right now. For the past week or so, I've been thinking to myself, "I wonder how Zoey's doing, where is she?". For the first time ever, I'm thinking of starting Progesterone and been in conversation with my provider about it. So we decided to possibly start in a couple of months. Been on HRT for close to a year and a third now, so I feel it may be time, with the idea of starting slow then building the process. It's a bit nerve-wrecking but also exciting.     I'm in California. As progressive as this state is, we don't have as many trans doctors providing general practitioner services and/or endocrinology for those of us that are trans. Dr Tomson gave me the best explanation I've ever heard regarding the use of progesterone and the variants. So informative. FYI, I'm 53 and have been on Estradiol Valerate and Spiro for 10 years. I had man boobs before I started, but they never really developed in all these years. I just had weight loss surgery, so I'm going to wait before I ask my endo about it. But I do have breast augmentation planned for late this year or early next.

    Ayo the gamer chair. Thanks for the info, it’s helpful since it’s sometimes confusing due to lack of readily available information. I’ve been getting ready to start HRT within the coming months or so, this topic is the only thing I’ve really used Google Schooler and local university’s research data bases.

      @@ZoeyBlack so far so good. I'm very pleased with the process as slow as it is. I'm sleeping better, but still take naps more than ever lol. Went from a sleepless maniac, to a cat. Went my entire adulthood with maybe 2 naps lol, I pass out like 3 times a week now 😅 (That's since start all HRT)

      @@ZoeyBlack I'm am soooooo much better with the anxiety especially. From when I came out to when I got on hormones took 10 years. In the last 5 months, I've enjoyed quietly sitting, contemplating. I had a pretty messed up childhood and for the first time in 28 years, I sing, I dance all day. I am happy. I still have a few issues in public but after all this time and therapy, my confidence grows.

    About to start my hormones next week and obviously interested in the use of and when to implement progesterone. I'm not seeking a "full transition" or anything gender affirming outside of hormones. As some would describe its a feminine tug as opposed to a need to fully express 1 way over the other.
    Gonna take more with my therapist and doctor before I start so yeah.

Yeah, completely get you about only wanting to do hormones, there’s so much pressure for trans people to “transition” and then also “fully transition”, which is so weird to me. So I’m glad you’re clear about what you want. And if that shifts or changes, I’m sure make a considered decision. So happy for you 😊

      Yeah I use to fall into that thought process "what are you transitioning to then" the answer really is "me" but coming from a Roman catholic conselfative upbringing...took a long time to deprogram.
      I've always been one to do things my way, yay for autism and oppositional defiance, so I'm not gonna let anyone's lack of understanding influence my decision.
      I don't identify as a woman just a much more feminine individual who wants some mommy milkers XD jk but yeah.

    In Brazil we have just PERLUTAN (ACETOPHENIDE ALGESTONE + INJECTABLE ESTRADIOL ENANTHATE) and Mesigyna (Norethisterone Enanthate + Val Estradiol 50mg + 5mmg) We ddon't have the opttion of taking estradiol injectable without progesterone, and is very common in our comunity take PERLUTAN... We have doctors, butt the most don't undestand or studies about transitioning, is vert expensive and hard to find...

      That really sucks that the doctors aren’t educated on trans healthcare. We have similar struggles in South Africa. There are only a handful of them that do hrt.
      Is PERLUTAN an estrogen? It’s the first time I’ve heard of it. What’s your experience been on it?

      @@ZoeyBlack I would say that 95% of us trans women make our own HT, Perlutan is a contraceptive composed of algestone acetophenide estradiol enanthate 150 MG/ML + 10 MG/ML . It sells in all pharmacies, without prescription, very important, image the number of unwanted pregnancies that are prevented in a country that does not have medicines for all... We, the Brazilian trans community, use this hormone a lot. There are practically no specialized endocrinologists, medicine is very sexist and with remnants of Christianity. Perlutan is defenitly the most popular drug in trans comunity here, off labbel.

      Wow, the community there sounds so incredibly resourceful. I mean you’re making your own hormones. I know it’s not the most ideal situation though. Thanks for sharing this with me, I’m always interested in what healthcare looks like for trans people in other countries. Do you know if there are plans from the Brazilian government to make gender affirming healthcare more available to the community?

    I've been on progesterone for a year and it's definitely changed the shape of my breast and aided the growth of my thighs and butt. I just don't know when to come off of it...
    I've been told to come off of it when I am satisfied with my boob shape, whilst I've also been told to not be on it for any longer than a year.

      Yeah it’s a little bit unclear and sometimes confusing as to what the timeframes and dosages are for best results. I actually might go on Progesterone and see if it’s doing things for me.

    Howzit Zoey, I was prescribed “Microgynon 50 ED” which, here in Australia, is pretty much standard HRT prescribed for MTF. It is a birth control tablet. Not knowing any different I didn’t question the oncologist and just went with it. I have done some digging around on various websites only to find that it is not “strong enough”. Doctors here are quite reluctant to give the injectables, maybe because of my age, I am 59 now and have been on Microgynon for the best part of ten years. Breast development sort of peaked to a full A cup and hasn’t changed for the last seven or so years. What is the “normal” HRT prescribed in South Africa? Taking into account my age and that I have had double orchiectomy 12 years ago.
    By the way I think you are awesome.

      Heya! Sorry for this super late reply, I don’t know how I missed your comment 🤷🏽‍♀️ As far as I know, the injectable doesn’t carry any significant risk with increased age. So I don’t know why they wouldn’t want to prescribe it to you. Are you doing it through the state or privately? Because I here in South Africa, the state provides pills and not the injectable, that you need to get privately. We’ve been trying to get the government to move away from the contraceptives as a form of HRT because it carries too many side effects, where as the bio identical estrogen is much more effective and safer.

      @@ZoeyBlack if it's like in the Netherlands, the only covered drug is the cheapest alternative on the market that's not contraindicated for the patient.
      Side effects are not considered unless they are serious enough to need treatment with medication more expensive than the next cheapest alternative that doesn't have those side effects...

    I cycle my progesterone to follow a typical menstrual cycle, half on and half off over a 29 day period. I would be interested to hear Anastasia's comments.

    What food are you supposed to take it with? She didn't say. And I tried doing it as a suppository for a while but I don't know if I was doing it right. One of my friends says to puncture the pill with a needle before putting it up there but I've never had a needle at hand. I dunno

      Oh snap congratulations! Um, best advice I can give is to take your time in finding the right surgeon. You want someone that your comfortable with and who will be able to deliver on the results you want

      I’m not sure about the prevalence but I know it’s happened to some trans women who have gone on Progesterone. Dosage is tricky with P. Really sorry about that. Have you spoken to you physician about it?

    Hi, I have spironolactone and haven't tried this it's my first time having it, and looking at the effects for it, I don't know if I want to take it. I read around is it best taking progesterone hormone, which I see what I want to achieve with my transition. And I don't have heart failure and high blood pressure, and I don't want to feel dizzy than I am and not pass out at work.

      You should definitely speak to your physician to see if this is something you want to pursue. You don’t have to be on it. I’m not and I’m happy where I’m at.

      She is in private practice. But she does a lot of work with the state in terms of workshops, speaking at conferences, lecturing, etc.

    been on 100mg daily of prog no issues yet, sex drive a little less but still pretty good. Idont have testicles they were removed. I won't know for sure the changes for a couple months or so. been on hrt for about year now doubled my estrogen from 4 mg to 8mg recently too. Boobs hurt alot more growth I'm assuming. thank got im not on spiro that drug is nasty as hell to be on!!!!!! omg! i was in such pain for about 7 months!

      @@ZoeyBlack update I still have zero sex drive i don't think that's going to ever change now. But my boobs are growing more since adding prog daily i have is 100mg prog and 8mg of estradiol no balls they were removed, having SRS planned in June this year 2024 my boyfriend is ED so this actually is not that big of a deal to me there are other ways so have intimacy besides penetration. Holding hands is a big one for me.

      @@ZoeyBlack Zoey I have 0.00 sex drive that's not a big deal for me, milage will vary person to person I am close to a C cup since adding prog to my HRT but again i don't have the ball things and having SRS 7/9/24 so i have to be on HRT men are not worth the bother so no sex drive is fine by me lol

    I definitely found this helpful. I've been taking progesterone for almost a month, and maybe "should have" intensely grilled my doctor about the current state of research on the subject, but... she kinda lets me follow my intuition without even keeping a close monitor on bloodwork, as long as the experimentation falls within reasonable limits and I inform her of any dosing changes. There can be a lot of psychological baggage to work through during transition, so that's something to bear in mind and try to stay flexible about. I backed off to only half the usual spiro dose, early on in my HRT process, because I had too long of a history of internalized trans-phobia to shut off so much of my own testosterone too quickly. It was dissolving my previous straight identity before I'd formed enough of a new one, so even though my body probably would have preferred more testosterone suppression (to resolve the "existential angst" depressive feelings that I experience as gender dysphoria), I had to go through more therapy and internal dialogue before realizing that. Gosh, this is all so complicated! Better than trying to kill the pain with booze and masking and emotional repression, tho! 🥲
