This is Gate 1 of the Thaemine legion raid on Paladin. This was done in Hard Mode. Normal mode requires item level 1610 and hard mode requires item level 1630.

Note that for the tentacle grab mechanic, this could also happen once Killineza has attacked the crater twice.
