Bro, what if severity is high,Occurance and detection low but rpn number more than 100, what to do? example, 8x4x4, how to do?

We have to take action to reduce detecting rating & occurrence rating (by providing poka yoke / mistake proofing) and try to bring the rpn less than 100.
