Sinus Arrhythmia ..and the drummer par excellence

- In this video, Dr Sanjay Gupta, Cardiologist discusses sinus arrhythmia and its significance. A full transcript is available on www.drsanjayguptacardiologist.com
Sinus rhythm means that the electrical impulses which regulate the heart rhythm are being produced in the pacemaker that god gives us all when we are born. This ‘pacemaker’ is otherwise called the Sino-atrial node and this is why this rhythm that is generated from the sinus node is called sinus rhythm.
A nice analogy is to think of the sinus node as a drummer who beats the drum at regular intervals. The drummer is receptive to what the body tells it and therefore increase the speed with which it beats the drum in response to adrenaline and slows the speed does in response to activation of the vagus nerve. When we are young and healthy, our drummer is extremely receptive and therefore our heart rate can go up quickly when it is needed to and slows down equally quickly when it is no longer needed to.
This responsiveness of our drummer is measured in terms of heart rate variability and a good heart rate variability is considered a sign (but by no means the only sign) of very good health. As we get older or sicker, the drummer becomes less receptive and our heart rate variability gets less.
Usually the drummer beats very regularly and if you measure the time difference between beats, it does not vary by more than 120 ms and therefore on the ECG, the intervals between successive beats look identical. Each beat looks normal and identical with a normal P wave (which tells us that the rhythm is being generated in the sino-atrial node) followed by a QRS complex.
Sometimes we can see that the time difference between successive beat intervals can vary by more than 120ms and because of this the intervals between successive beats look different on the ECG. Each beat still starts off with a normal P wave and is followed by a QRS but the time interval between 2 P waves can vary by more than 120ms. This appearance is termed sinus arrhythmia.
The reason this happens most commonly is because of breathing. When we breathe in, our vagus nerve is inhibited and therefore our heart rate increases and we breathe out the opposite happens and because in young, very healthy people, our pacemaker is so receptive, we see this phasic change in the time intervals between each heart beat by more than 120 seconds which may then show up as a slightly irregular rhythm on the ECG. Often it you repeat the ECG during breath-holding this pattern will disappear. Sometimes sinus arrhythmia pattern on the ECG may not necessarily be linked to breathing but nevertheless it does not signify anything to cause concern in the overwhelming majority of patients. In my 30 years as a cardiologist i have never come across a single case where the sinus arrhythmia was telling me that there was something sinister going on in the patient.
Sinus arrhythmia is simply an ECG pattern. It will cause no symptoms and does not have any bearing on a person’s quality of life or length of life. Moreover, it is a sign of good heart health and a healthy pacemaker. As we get older it is common for sinus arrhythmia to become less prevalent on our ECGs.
It does not require any further investigations and does not require any treatment.
The only Doctor that makes sense and total clarity on cardiac issues.
@تلاوات القران - Tilawat Al Quran What is different between cardiac and sinus?
You are the reason I stay sane with all that I deal with my heart. Thank you from the bottom of my heart for doing these videos and for dedicating your time to educate us so clearly. Thank you 😊
You have been such a godsend. Watching your videos has taught me that a lot of my PVCs are the result of tenseness. When I concentrate on relaxing my 'core' muscles (stomach, groin, etc.) during an episode, the PVCs go completely away. Thank you so much for all you give away so freely to help those of us who live in anxiety about our hearts.
I was diagnosed with sinus arrhythmia yesterday.. This reassures me a lot Doctor Gupta, thanks a lot!
Diagnosed? I sure hope we all have it. If not, likely the vagal trunk to the heart is cut and is stuck at one speed.
Me too bro. I know your condition.
Thank you Doctor i am 27 with this sinus arrhythmia and i was very worried cause most of the day i was on sinus arrhythmias ❤️thank you for the information

Thank you so much for this Dr. Gupta. You are a God send to many. Keep up the great work!
All your videos are very helpful. I feel like a have a doctor by my side helping me all the time. Thank you very very much.
Hear conditions run in my family and I'm always constantly worried about my heart health so most of my life I thought my breathing related heart rate changes were a problem. But you and my primary care Dr both say this is harmless, and a good thing! Very good to hear!
Thank you for posting on this particular topic. I have dysautonomia along with sinus arrhythmia and have had these conditions all my life. I have often wondered if it had any significance throughout my life with having sinus arrhythmia. This has been very reassuring! Thanks again!
How old are you?
I feel a little better after watching this video. I am 73 year old woman who has been diagnosed with Sinus rhythm, T Wave abnormality (II, III, aVF, V4, V5, V6). The cardiologist said that arrhythmias are common and my condition is probably nothing to be concerned about. However, he added that if the symptoms become worse I should go to ER. This last bit worried me somewhat. Now I find myself monitoring myself for symptoms. 🤔
Another informative video, thank you! I've also had sinus arrythmia show up in my ecg. Usually it's when I rush to do my ecg and don't have enough time to sit and calm down for a few minutes. When my heart rate is a little up, that's when sinus arrythmia appears.
Exactly
Thank you Doctor Gupta! Your by far the most informative and helpful cardiologist i have come across. Again thank you for taking out your time for making such useful videos, i usually watch for my mother whom suffers from high bpm. Thanks again Dr Gupta 👍
You explained sinus arrhythmia really really well,
Better than any one I’ve listened to on UA-cam
Most cardiologists make it sound difficult ,
Now I get it now, I understand because of how well you’ve explained it 🙏
Once again, thank you for your video. I am continuing to take a magnesium morning and one at night. I really feel it has helped me. Have a great day..
Hi doc. So I was watching a series here in the US. It’s called The Resident. One of the actors who plays a doctor said he wanted to be well known like Sanjay Gupta. I thought that was great.
What episode? I love the Resident
502JoJo sorry I don't remember. It was a long time ago. But also I heard there's another infamous doc with the same name pronunciation but here in the US. I'm not sure which one they were referring too.
There is another Dr Sanjay Gupta. Have you got the right one. This Sanjay is not in USA
They might have been talking about the American Sanjay Gupta who is also a cardiologist who appears on CNN 😁
Thank you for another informative 'seminar' that helps to put us at ease.
He is the best... I started taking magnesium and i do feel better!!
Thanks doctor for posting such a informative video about SA. It will rule out all myths about SA. I have been following your all videos from quite long time. Recently I am hearing heart beats in my ears (left ear only) during night time , done with all tests & finally showed Sinus Arry. Doctor said normal but I was still concerned . Then came across your video about this which made me to feel better now . Thanks again .
Do Echo test as soon as possible

Wow I had a doctor who I told I had a sinus arrhythmia and she freaked out . I figured I had a decent ticker seeing as I was a mover for several years where I was active daily and was younger at the time so good to know thank you
Dr. Gupta, you're much better than your American counterpart on CNN! I found you trying to research hypertension in pregnancy and preeclampsia. There is very little good content about this, and this is a very scary condition for pregnant women. If you are able to share your perspective, I think that the general community could benefit greatly.
wow well explained sir..."heart rate variability".......that is why young people can handle strong emotions so easily, bcoz of their super receptive "pacemakers" like they are superhumans. My heart is not so receptive anymore its hard to calm down once my heart beaten up......I envy them
thank you! you have put my mind at ease.... I do have that related to breathing. Now I know it's no big deal...
Man. Thank you. This was incredibly informative. I have an sinus arrhythmia and have been having alot palpitations lately so I get worried it's something harmful. This is reassuring.
Same man when I suddenly breath out the next pulse comes a just little late . I was worried about it . When I breath normally it rarely happens.

In the mean time I research alot and stay aware of my body♡ Thanks for sharing this made me smile

My daughter who is now 13 has quite pronounced sinus arrhythmia but I have been reassured by the doctor that it's normal. It just feels very odd for a non medical person to feel her pulse and I was worried it was abnormal.
I have had the same thing with my 9 year old daughter I can feel it in her pulse too, she also feels it as a tickle in her chest.

This Doc is the best... doing these videos for all of us to have access to. Super informative, and super helpful. I have strong Sinus Arrythmia... I have actually heard that with a breathing sinus arrythmia can be a symptom of a very healthy heart and good health. Before I knew this I was freaked out when my doc said after an ECG... You do not have a normal heart rate... LOL but I had sinus arrythmia.
What was your heart rate ?
I have sinus tachycardia that comes on for no apparent reason it’s gotten better with medication and me understanding it but when I first started getting this it was so scary I’m a anxious person as is so then in turn that made it even worse in my eyes but it still scares me I have watched so many of your videos that have help me try to stay calm and try to think my heart is not gonna just stop I have had lots of test and they say my heart is healthy but still In spite of all this I still get really scared sometimes and I don’t know it just sucks sometimes but thank you for all your videos please continue to make them they have help me more then anything I think

Thank you for the information, is there a link between breathing in and inducing ectopic beats? Often people having a deep breath in can sometimes cause the heart to skip a beat, is that still releated to the vagus nerve? Thanks again

I just got diagnosed with it. And it was causing me really bad anxiety, and it was hard for me to breathe. It's still slightly is a little tight in the upper chest most likely because I have been unsuccessful at quitting smoking so far but im trying
I understand you! Have exactly the same feeling and I really am wondering if it is because of anxiety or because of arrythmia.

How dangerous is a sinus arrest?
Could this lead to death?
I experienced a four-and-a-half second pause and wondered what the possible consequences might be for a longer pause.
I am an ordinary citizen trying to come to grips with his predicament. The installation of a pacemaker has been recommended but I am resisting. I am 72 years of age and have undergone triple bypass surgery a few years back.

Magnesium chloride cleans blood vessels and therefore prevents and prevents heart attack, stroke, and many other blood circulatory problems, such as arrhythmia as well. For it removes the fats from the blood vessels and impurities inside them that clog them, unclogging them, and thus causing the blood to flow freely through the body. But you can't take it forever, you have to take it for a while and then stop. Hope it helps everyone and hugs!
Thank you doctor!
Dr. Gupta, would you do a commentary on how to treat patients in persistent/permanent atrial fib? What are the consequences of staying in AF and using a DOAC, and what are the consequences of doing an a/v node ablation with pacemaker (also on a DOAC)?can either scenario cause congestive heart failure faster than the other?

Hello Dr Gupta !
what if sinus arrythmia with almost a 45 beats per minute when lying down only using an android phone.
Experience fatigue, sometimes chest pains, and weakness.
Thats completley normal thats usually what i get sometimes its whe your hearts beating to slow combined with the sinus arythmia it starts to cause slight symptoms dont be to worried about it though
Totally same here.. when laying down my heart rate slows down to 54 bpm.. and experiencing chest pain everyday, sometimes not able to walk (feeling like wanna collapse)
But i had done 4 ecgs, 2 stress test and even ct scan calcium score and all turns out normal
I am thankful for that as Doctors in the UK are frightening patients into taking pills when it looks like it is healthy for the heart to have irregular beats. What are some Doctors causing patients in the UK, frightening patients into having a heart attack?
What if you have sinus arrhythmia with ectopics beats?
Dr Gupta, I have had ECG in the past and it just showed sinus rythm. For the past 1 year, I discovered that I have GERD with Hiatus hernia and develop episodes of palpitations and panic attacks. I had fatigue and low effort tolerance and found out that I have iron deficiency as well. Had iron infusions and now the fatigue has improves. During those episodes of palpitations with frequent visits to ER my ECG just showed sinus tachycardia. However at rest I noticed that my heart rate goes down to 50 something only while previously it was around 60-70. I had Holter done twice and it is considered normal. For the past 1 week I feel uneasy to sleep and 1 night waken up from sleep with left sided chest pain and sweating. I had ECG done for 2 days and the dr noticed sinus arrythmia this time. Can a previous normal sinus rythm ECG which evolves to sinus arrythmia still be a normal variant?
Haitus hernia would causing all this problems. bcoz in this condition some upper part of stomach go through the chest cavity leading to this prob due to bulid of pressure on vegus nerve between stomach n heart junction. Plz do your haital hernia surgery and once correct this problem n see all heart releated issue disappear with time for sure.
Cheers Sanjay, you don't mind if I use your style and vids to teach our students and refresh my colleagues? You very helpful and good communicator indeed, well done Sir

IS it the same as sinus tachycardia? I just had an Ablation 3 mths ago with now I have tachycardia. 😰😰😰😰 and low blood pressure at night. Resting heart rate is 95. I always wake up in the morning HR 107. If I can get my BP up it seems to help my heart rate.
Hey doc!! how do i know if im having panic attacks causing palpitations OR some type of arrhythmia issue??
I'd like to know too, it happened to me a few nights ago. Nothing new though. Edit: having now watched the video, he mentions breathing. A panic attack affects normal breathing, especially when the heart speeds up. So this may have something to do with it. The 6 3 12 3 method can help, 6 in, hold 3, 12 out, hold 3, repeat til heart rate steadies. Not easy though when heart is fast but it does help me. There are variations on the numbers.
Usually an arrhythmia will best irregular or itll suddenly start beating oddly fast in a split second out of nowhere. If your having a panic attack then your heart starts pounding over the course of 5 10 seconds it's most likely sinus arrhythmia like he says In this video
@@XxToxicVenom5xX i feel panic and then it takes over and my heart races... my heart never races without me feeling panic so idk
@@BrotherDave80 sounds to me like it's just the adrenaline causing your heart to pound once you panic. That's a very common and healthy response. As long as it calms down once you do sound fine to me.
@@XxToxicVenom5xX it usually lasts anywhere from 5 mintues to 30 mintues but yes it eventually calms down

Do you get skipped beats when sleeping on the right side ?
Wow. I was just wondering if sinus arrhythmia and sinus tachycardia were the same things
no sinus arrhythmia and sinus tachycardia are two different things actually with sinus arrhythmia you may have like some skipping beats but that's pretty normal with everyone everyone has some skip beats every now and then I'm just that some people can fill them in some can but that's nothing to be concerned about but sinus tachycardia is a different situation usually the beats are prolonged and they last for over a minute and they range from like 102 the 150s but supervision to supraventricular tachycardia your heart rhythm is like 160 to 200 in like 20 or 30 and that can be deadly if not taken care of
hi i got and ekg and an echo scan and they were both normal but i feel my heart beating fast all the time my heart rate is always a little faster than normal and now i’m wearing a heart monitor if the other 2 tests came back normal do i need to worry about having tachycardia, i’m an anxious person but don’t have like any symptoms
