Sir another technique for placing caoh is using paper point and leaving the paper point there only. Cut it. Place teflon then temporary. I found it very easy and effective. The paper point takes the caoh till the apex.
Yes, you are right. This technique is easy to use and can be used in straight single rooted teeth. In case of curved root canals the removal of paper point may be quite difficult. It may get stuck inside the canal. Also if the scheduled appointment get delayed paper point may act as a nidus for reinfection. Anyhow in many cases it can be used with less effort and predictable outcomes. Thank you

With both materials make sure that small extensions are not exposed to oral cavity. If packed properly both are good materials

We have to understand the following in case patient reporting with swelling A. The size of the pulp chamber and root canal are large B. Mostly this would be the first time the tooth becomes symptomatic (in case or large swelling. If it was persistent, there would have been a sinus tract) Then, 1. Do a thorough cleaning and shaping 2. Sodium hypochlorite must be used 3. Fill the root canals with calcium hydroxide 4. Recall on the third day and replace the calcium hydroxide 5. Advise antibiotics (Metronidazole must be included) Hope this is helpful

And also when we use the master cone to compact the calcium hydroxide aren't we extruding it in most cases?
1. If no intracanal medicament is placed, minimal number of micro organisms which are present after cleaning and shaping would multiply and reestablish an infection. So the canal should be never left free 2. Mastercone usually will not cause extrusion of intracanal medicament as the consistency is thicker compared to a sealer. Sealer may extrude, but not the intra canal medicament as the flow tendency is less unless we are using very positive pressure to compact

Sir,In large abscess cases,after giving oil based calcium hydroxide for 3-4 weeks.can we remove the calcium hydroxide and obturate it instantly .If we keep this another 3-4 weeks( if it is not healed properly ).Is there any problem,please explain sir.
Calcium hydroxide when placed more than 21 days could weaken the roots. This could later leads to fracture of the roots. May be upto a month it is okay. More than that I do not recommend

Exactly. Air in the canal get entrapped inside leading to incomplete canal filling and also pain. This can be easily avoided by injecting calcium hydroxide half way through the canal and then using master cone or lentulospiral or rotary file in reverse direction to aid in filling the entire canal.
Betadine solution can be mixed with calcium hydroxide to increase the efficacy of calcium hydroxide against E Faecalis. But the duration of action is short lived and some iodine products can cause severe staining of tooth. So preferably it can be avoided
The alkaline pH of calcium hydroxide may cause the dentinal structure to collapse owing to the destruction of carboxylate and phosphate groups.
