      Thanks 🙏🏾. Very educational but I think the question no. 30 should be checking the client IDENTIFICATION BAND because if the client is confused and disoriented, she might mention any name for an answer. In all, I remain very grateful for your efforts. Thanks.

    Never saw exams easy like this into the nursing program, exams are completely different. They make it hard as hard. Don’t ever think it is like this. You can earn an idea, but No exams look like it.

      exactly, they are way harder. I don't even have to think to answer these questions. Nursing exams make you go into parts of your brain you didn't know existed.

    Please review question number 7. That's the least as it's a form of constraints. Also, the question did not indicate any symptoms that would prompt elevating the side rails e.g seizure, agitation, etc.

    Can someone explain question 27 to me please. Is there a conversion? I looked online and I found that 5 ml or 1 tsp is equal to 100 drops. I cannot figure out why the answer is 60 drops.

    Can someone explain # 4 ? Why would we lift the pt by the forearm? Cant we injure the patient this way?

      Nvm, i see now we never bend at the waist, so this cancels that question out. The last two make no sense to assist the patient. either way, i don't think we should lift the patient by the forearm

    #7 is incorrect as well. Leaving the side rails up is a type of restraint and would be considered false imprisonment

    I'm confused about question 30 because the answer is "ask the client for their name"but I was taught to look at their identification band as some patients won't be able to tell you their name

      Depending on what setting, because some places don't have I.d. bans or the patient may have removed it🤷🏿

      The best way to get ID is to have the patient tell you their names. You can double check the wristband. During trainings in many hospitals I have had, it's always to ask the patient.
