It's a reference to a play by Shakespeare called Macbeth where a character called Lady Macbeth feels guilty from murdering someone. Her guilt is expressed in the form of a spot on her hand that she cannot get off of her hand. The joke is that this is a very casual re-enactment of what was originally an extremely dramatic scene.
