Zancrow died before he was going to take out Medly for betraying the guild and Zeref woke up from his coma and blasted them with his curse. That being said I still think he probably wouldn't just be lying there; since this was still Zeref back before he finally decided to commit into his public perception, I always assumed he would've given his accidental victims a proper resting place, even if it's unmarked.

I've always wondered about Erza's partial immunity to illusions magic. But it does rely on her being aware something is an illusion, then she shuts her left eye and see through her right artificial eye to see through the illusion. If she had realised it, Erza might have been able to warn that illusion magic was being used.
