All the chords should be broken, some more than others. The fingers need to point downward on the strings, the tips as low as possible. When they are above the wrist and you try to bring them away, you crimp your wrist and that easily leads to tendonitis. Always let them dangle down and close down into your palm. That will remove any problems and give you more tonal control.
I wanted to play them all broken actually but the exam score I had, shoeed only certain chords written as broken. It does sound much nicer with them all broken!! As for hand position, yes I have developed this hideous habit of placing from the 4th and thus my fingers are much too high. Well spotted. Which is why, these next 3 months are being spent on conditioning exercises to rectifying poor technique before it becomes impossible to fix. Also, don't play a tempo any faster than you are comfortable with in the most-difficult place. Don't let your ear dictate.
