  • In this video Dr. Kamal Singh speaks about diabetic ketoacidosis, it's symptoms, how to diagnose it, and its management. Diabetes ketoacidosis (DKA) is a serious and life-threatening complication that affects people with diabetes and those who have undiagnosed diabetes. It occurs when your body doesn't have enough insulin (a hormone that's either produced by your pancreas or injected). Diabetes-related ketoacidosis can develop in people of any age who have diabetes or undiagnosed diabetes. Early symptoms of DKA can include frequent urination, extreme thirst, headache, dehydration, high amounts of ketones in your pee or blood and high blood glucose (blood sugar) levels (over 250 mg/dL). More severe symptoms of DKA can include nausea and vomiting, abdominal pain, fruity-smelling breath, shortness of breath, tiredness, weakness, decreased alertness, confusion and disorientation. The main cause of DKA is not enough insulin in your body. These conditions can also contribute to developing DKA: Illness, infections, emotional or physical trauma, pancreatitis, heart attack, stroke, alcohol abuse, drug abuse, intake of certain medicines or drug abuse. Diabetes-related ketoacidosis is diagnosed if you have the following four conditions:
    1. Your blood glucose (sugar) level is above 250 mg/dL.
    2. Your blood pH is less than 7.3.
    3. You have ketones in your urine and/or blood.
    4. Your serum (blood) bicarbonate level is less than 18 mEq/L.
    Treatment includes IV fluids, insulin and management of symptoms. Risk factors for developing DKA include family history of diabetes, family history of autoimmune diseases, poorly managed Type 1 diabetes, or poorly managed Type 2 diabetes. If you already have diabetes, there are many things you can do to prevent diabetic ketoacidosis, including checking your blood sugar often, taking your insulin and/or medication regularly, checking for ketones, seeing your healthcare provider regularly, and staying educated. Most people recover from treatment for diabetes-related ketoacidosis within a day but sometimes it takes longer. If not treated soon enough, diabetic ketoacidosis can lead to severe complications including very low potassium levels (hypokalemia), swelling inside of the brain (cerebral edema), accumulation of fluid inside of the lungs (pulmonary edema), damage to your kidneys and other organs, coma or death. If you experience symptoms such as frequent urination, extreme thirst, high blood sugar, ketones in your urine or blood and vomiting, call your doctor immediately.
