For people that are struggling, there is an item behind the bush, press the rat and you get a... uhhhh... idk but its a rat in a cone. And he has a pattern as well, which is blaster, bonewall and bonecircle and then bonezone, i recommend you to jump after the blaster so that the bone circle is in the air and misses you, for the bonewall just dont go to near to sans, and for the blaster its the same.

If the normal 3DBB sans fight is too hard for you, at 2:38 (as soon as he respawns), right under the hp bar, is a plant. By the door on the floor, there is a rat. Touch it to get a [Ratcone] healing item that heals 30hp.
