Could that sinus beat before the PVC represent a situation in which a "concealed" reentrant beat is emerging but never exits to a full-fledged complex but alters local mechanical function enough to manifest as partial dysynchrony? That seems more plausible than an autonomic influence fortuitously appearing immediately before the PVC.
