I think you should have ground deeper before moving forward. Using mainly the front of the grinding wheel go to full depth before moving forward. Then return to the top of stump and inch forward, again grinding to full depth before moving forward. I’m picking up a stump grinder this afternoon; I’ll make some videos.

Probably 3 to $4 per inch
My son operate a stump grinder and he charges$30 per square inch in Charlotte❤
Up and down are not backwards. You move the level in the direction you want the cutter to move around it’s pivot point.
