Cousin does not mean one specific relation, but more like a whole category of farther relatives. And far-enough cousin is fair game, though it may not be obvious today.
Think about it, for millennia people had bigger families AND traveled less than today. There just had to be some blood relation. The taboo is in place so that the eventual children are born healthy. If your cousin is 3-removed or farther, there's no *medical* reason not to go for her. Or him.
