She was named that by her producer Billy Sherrill, when he told Tammy she was the first lady (singer) to ever sell one million records, in around 1969. Then her at the time husband George Jones said he wanted to name their next album "Me and the First Lady", which they did. I love Loretta, there will never be another Loretta.
