  • What is a laminotomy?
    A lamintomy is often confused with a spinal surgery called a laminectomy. While these procedures are very similar, there are some important distinctions.
    In this video, I'll show you:
    - the difference between a laminotomy and laminectomy
    - the most common diagnosis' that may lead to a laminotomy
    - what you should consider before having a laminotomy

    My brother in law just this AM had his 4th or 5th laminotomy. He has had numerous lumbar surgeries as he had polio as a child and was left wearing a brace on his R leg. He has undergone a spinal fusion as well as laminectomies. This poor guy is in pain all the time! His gait is SO far out of whack, but he refuses to use a wheelchair. I think he's doing more harm than good, but he was raised to be independent and just can't let go of that mind set. This was a great video. You explained things beautifully. TYFS!

    Thank you so much for this Video, I used to be a Medical Assistant and before becoming a Medical Assistant I was a Nurse Aide and Medication Aide at Nusing Homes etc, I honestly don't know what happened to my disc issues besides lifting on patients wrong and maybe from my abusive relationship that I am no longer in Thanks to God but back in 2018 I was told nothing could help except surgery and I decided I wasn't going to do surgery because of so many bad reviews on Spinal Fusion but now here in 2022 it has gotten so bad I can barely walk for more than a few minutes before I have to sit down so I am waiting to get an EMG test done and surgery hopefully soon doing I believe a lamenotomy.     Thank you for explaining both the laminectomy vs the laminotomy in layman’s terms where they are easily distinguished from one another. I was recently diagnosed with cervical osteoarthritis in my neck (I had never heard of this as I come from a medical background) and I have been having radiating pain up and down my left arm as well as occasional pain tingling in my fingers sometimes and the pain had gotten to where it is nearly impossible to control. So if the physician recommended treatments do not work I may consider the laminotomy as a last resort.

      Hello, there are quite a few indications for the recommendation of a laminotomy. Neither one is necessarily "better" than the other. A laminectomy typically has more bone removed than a laminotomy; however, it's a bit of a continuum. Some surgeons may interchangeably use the words and technique based on their training and experience.

    What would be the advantage of doing a minimally invasive laminectomy instead of a laminotomy for spinal stenosis?

    There is another procedure to help with Spinal Stenosis, it's called the MILD Procedure. This is an acronym for Minimal Invasive Lumbar Decompression. I'm sure you have heard of it. What are your thoughts on this procedure. I think they still remove some of the lamina and tissue causing the pinching of the nerves. But I was told it no more invasive than an epidural.

      I'm very familiar with that type of procedure after practicing within a minimally invasive surgery practice for many years. My personal opinion is that while minimally invasive surgery is often "better" than more invasive procedures it is still a surgical procedure and comes with nearly all of the risks and issues associated with surgery.

    Hi Dr. Thanks for the video - you explained very nicely. I had a laminotomy in South Africa 6 Months ago. I.have made a good recovery, however I do feel different. Still some pain, some tingling, feels like something is almost missing there lol. My question is, am I able to start training again - mainly rowing? All I have done for 6 months is walk 5 km a day as I have been too nervous to row. Thanks you!

      Thank you! I'm glad you have found relief. In terms of limitations, I would consult with your local healthcare team. In general, most people are able to get back to most activities by the 6 month marker, but you'll want to check on your individual restrictions with your health team.

    I have had a fusion, laminectomy and now a spine stim implanted. Now I have severe stenosis and numbness, Is the lamnotomy a good choice?

    Can a laminotomy lead to spondylosis?

      It "can" but doesn't every time. There are a lot of factors; however, anytime a facet (for example) bears increased load due to changes (tissue removal in the case of a lami), the ipsilateral or contralateral segment will naturally sustain changes due to the biomechanical and structural changes - which can potentially accelerate spondylosis.

    Why would a laminotomy be done on a three year old boy? What would be precautions to be take for such a young patient?

      The only cases that I can think where a laminectomy/laminotomy would be performed on someone that young would be 1) Acute Trauma or 2) Severe Congenital Defect- but it would still be considered a last option

    Does scar tissue fill the voids for the less invasive bone removal option once it’s removed to help provide longterm stability??
