    Many people with disc herniations follow a classic presentation: back and/or leg pain while sitting, bending, driving, lifting… basically all the things that bend their body forward as this is said to “push the jelly out of the donut,” increase disc pressure, and make things worse. And these same people often mention that standing and walking feels much better.
    But what about if you hate standing and walking, but love sitting and bending?
    Is that weird? And what could it be?
    Well, it could be several things…
    But in this video, I share with you a specific type of disc problem that actually presents in the complete opposite way of most disc issues…
    And so this video is perfect for you if you have little to NO back pain, but experience butt/leg pain that is worse when standing and walking.       This video comes the closest to describing how i feel. But I actually do have back pain. So is what you described in this video what I have? Where do I have stenosis? (I have numbness in my right shin and some weakness in my right quadricep which is improving.)

      @@williamkohrman1033 hey! I’d need some more details to confirm- but consider that stenosis is simply a term used to describe a narrowing of the space where the nerves travel. In the low back this usually occurs in a place we call the foremen/lateral recess. Both “spinal stenosis”- usually consider to be due to arthritis/degeneration AND disc issues can cause stenosis or narrowing of where the nerves travel. A disc can cause stenosis if broad/large enough to take up space within the lateral recess/foremen- in which case… as you’re experiencing, symptoms may present VERY similarly to degenerative causes of “spinal stenosis.” One differentiating factor is that disc issues usually cause an inflammatory presentation (positive/yucky straight leg raise) vs degenerative conditions which cause stenosis are more compressive (negative leg raise) in my experience.
      I believe on another video you mentioned leg raise was not painful during but a bit after… I’d consider this in general to be negative.
      Age as well as other history would play into distinguishing between what might be the cause here.
      That being said- treatment would follow a very similar process if looking to heal naturally 👍

    This video described my pain. I had right glute pain that extended down the back of my hamstring and stopped behind my knee. I had yucky leg raise and yucky back bend. Sitting down felt good, walking felt bad.
    Sitting in a chair and leaning forward is a simple stretch that has healed me. If you do these stretches, I’d recommend standing up and walking to see if it feels better. I was able to take 5-10 steps without pain and then the pain would return. So I kept at it and I was able to walk further and further as the days and weeks went on until I was healed. Charlie, you make this so simple to understand and I applaud you for it. I’d still be doing cobra poses thinking I need to grind through the pain if it wasn’t for you.

      @drcharliejohnson sitting always feels good, able to extend lower leg, no problem. Back gets tight and sore from standing for extended periods such as waiting in line at a theme park, browsing slowly in a store and things like that. Interestingly, my back usually feels okay while walking quickly, even fast hikes with a rucksack. Sometimes get pain and stiffness in left hip as well. I'm thinking disc issue?

    "Work on fixing your yucky motion, not your yucky image". I have had the mindset of fixing my yucky MRI and it also caused me to deal with depressive moods...and I'm a positive person. My orthopedic surgeon said that 90% of people in my condition will eventually need surgery. Meanwhile, I have 2 chiropractors that are telling they are not worried about my MRI as much as how I'm feeling. One of the chiropractors even told me that he didn't think my image was that bad anyway. He gave me some PT exercises and is focusing on my body's movement and my pain has improved dramatically. Basically, the positive outcomes from all 3 doctors are all in line with what Dr. Johnson is trying to illustrate with his program and videos. I hope I never have to work with Dr. Johnson :) but if something else comes up, he will be the first place I start.

      Of course the surgeon said that, cause 90% of the time he gets a paycheck as well. Also dealing with this right now as well. We will heal. Good luck 👍

      @@drcharliejohnson thank you still trying to figure it out. When I do the slump test both sides are negative. I have some pain in my calf and where the si joint is located at times but mostly when walking or standing for a while and some tingling in my left foot between big toe and the toe right next to it but only when I stand. When I start walking it goes away. Also the tests where you turn and lean back are also negative.

    This is best video on youtube about this problem.All others pt shows yucky cobras and simular things.But what motion is good for this simptoms ?

      Hey! Need to test and find out. That’s the fun of it. Many options = many chances or opportunities for relief. But need to test, treat, and retest systematically to discover what works for you!

    This is my story from my last two and half months. As I followed what the doctor and physical therapist were recommending I was getting worse and worse. Although both did finally conclude that it was likely a back disc problem without using an MRI (which has now been confirmed through an MRI and actually two disc bulges). But it feels like the PT has still been recommending exercises that haven't been helping me like push ups although modified etc...and I've been getting worse. I am scheduled for an epidural in two weeks, so I can "continue" physical therapy after because up to now the physical therapy and medication has led to no improvements. I really struggle to walk or stand for more than a few minutes now so over time I've just been getting worse. I feel best when sitting (with poor posture mind you) or lay on my non painful side but with my knees up. But I also have an additional problem and that's as my pain and symptoms have increased I've developed a lateral pelvic tilt. So when I do stand up, I'm crooked. And again the exercises recommended for the pelvic tilt just put me in more pain and the cycle continues.
    Hopefully the epidural helps, but I really feel like both the doctor and PT did the "standard" options and now it almost feels like they are just taking shots in the dark and hoping for the best. I know the PT at least has said he's consulting with others to try to come up with exercises that help arent to painful. But it's all kinda frustrating.
    I seem to be getting so many mixed signals. And the exercises I've been prescribed or looked up online do not seem to help.
    Hopefully the shots work and allow to start living my life normally again. And the PT finally can come up with a program that actually helps.
    So what are the types of exercises that help in this scenario? I obviously can't walk it off. And will it help with the now lateral pelvic tilt?

      Ahh man- sorry to hear Christian!
      Common that a general approach by general practitioners (PT/docs) aimed at treating the diagnosis vs. you (your pain experience) backfires and makes people worse.
      Honestly, this takes a dialed in approach, and would be tough to prescribe exercise/advice blindly without knowing all the details. A bit of a process I hope you can understand (and it goes well beyond exercise by the way).

      @@drcharliejohnson I will definitely take that offer into consideration. I have gone this far down the rabbit hole with the doctor and PT I've been seeing so I suppose I still want to give them a chance. As mentioned I already have a shot scheduled for a little over a week from now as the next step the doctor recommended. And I'm aware of the mixed results with them. And I am hoping for the best. I don't necessarily blame the doctor or PT but as I mentioned and as you mention a bit in your video, seems like they took a very standard approach which may have led me down the wrong path. Your video definitely reflected a lot of my experience though.

    Hi. In this video you describe my pain which appears when standing and walking (without any back pain). My physical therapist is treating this as a back issue, but I’ve seen no improvement. Is there a specific exercise to target my pain (my MRI shows L4-L5 broad disc protrusion).

      Hey! thanks for watching! To be clear- when you move your back- you are able to reproduce your symptoms... is that correct?
      If they are treating your back and you're not improving... then they need to either A- change their treatment approach (different area/angle) or B. or focus on optimizing your healing environment (mentally/physically first) before "treatment" or exercise prescription.
      If using motion to resolve this problem- goal is to see an IMMEDIATE change (no waiting and seeing) and remember to stay scientific. Most folks will only need 1-2 motions to resolve their pain on the exercise front.

    I didn't have back pain but failed the backward bend and right backward twist, now my low back is stiff and burns. I guess the dr. blaming my hip was wrong. I've had groin, hip thigh glute pain for a year, not sure where to go from here.

    I know how to avoid the pain by not turning to the right and bending a little as long as I don't do that I don't get the pain but do you want us to move into that pain and exercise somehow I wasn't sure of what you were saying

      Hey Rose! So, in the short term it may make sense to avoid yucky motions. But the ultimate solution is to correct your movement experience so that this motion is no longer yucky. Many ways to go about doing this.

    When I started watching this, it was Hallelujah, up until the diagnostic tests, which didn’t seem to aggravate it. Everything else was true up to that point. The feelings in my foot never go away whatever I’m doing, the tightness and discomfort are constant. However, I continue to try and listen to my body, and to what it wants me to understand.

      Hey Sandy! Dr. Charlie here- sorry to hear about what you're dealing with. Thanks for checking out my content.
      Takes a process (not a quick fix) to resolve this issue. It involves doing some detective work to isolate what's contributing to the pain (mentally and physically), establishing a clear baseline and understanding of what makes things better/worse, and then systematically testing motions to find which ONE will aid in resolving your pain (not a sheet of exercises). Then, learning what you need to do to get up to full speed systematically (so you reduce risk of re-injury and have preventative skills needed for long term success).
      Hope this makes sense.

    In my case , extension is killing me but walking not . Bending don't work too really . So 3 years with a sciatica in right leg and no solution for now :( .

    Press ups do make me worse (circumferential disc bulge AKA flattened disc/disc height loss)

      Common. But not common for symptoms to last forever. Are you still experiencing symptoms?

      I experience almost no pain (it got better slowly) but it's because I limit myself. Sciatica almost never there but it's because I limit my amount of sitting like I said. Sometimes the disc itself is painful if I sit about 45 min or an hour@@drcharliejohnson​

      Hey! What do you mean by trying to TMS thing? It’s not really something you try- it’s something you commit to when you have substantial evidence to suggest without a doubt that your pain is originating from the mind.
      Feeling a set back coming is just what you said- a feeling. And, cool thing is, you ultimately control how you feel 👍. Find safety and comfort… lean into a positive experience now and don’t give it power.
