When you tend to always act stupid and loud and happy for so long, that is engraved into the heads of others as being the “real” version of someone since us humans desire and think happiness as the best emotion. We never really think that many are simply hiding behind a smile as a facade for their anger or sadness. That, and when children are told by their parents to not interact with someone they perceive as a threat but really isn’t one, those children then start to ignore that person or people as well and don’t notice the suffering. On Naruto not having parents, it could be reasoned that the other adults told their kids that Naruto was a monster who killed his own parents or something like that. No doubt Naruto also probably himself believed that he didn’t deserve parents either. I mean, he questioned who they were and when 3rd Hokage told him not to bother or worry about that, he assumed his parents died due to disappointment or they did something bad and were killed.
Shikamaru, Choji and Hinata and in some cases Sasuke, were the only kids to notice his suffering. It’s only for the video that the author made Sasuke not care.
You have to think that no one even knew that he was an outcast from everyone. Also Naruto's always happy and upbeat and the timeline of them at this point is that I don't think others would realize that you can have a REALLY hard life and still be so happy if you learn to get over it.
