    My psa was 3.5..on my yearly blood test..it went to 13 following year..so i went for mri and biopsy..was told i had aggressive cancer gleason 8..i wish i went for psa test in 6 months..i had no signs and was very healthy...had surgery and radiation..so far im 4 years cancer free...64 years old thats my story

      You give me hope. My PSA was recently 3.94, jumping from 3.07 just 6 months ago. Going for biopsy on May 16th. I DO have BPH, which may partially explain PSA velocity, but either way...... early detection and treatment is key. Thanks to ALL of you here.

    interesting conversation...my PSA was low up until a year ago (64, very active lifestyle). This past physical my PSA doubled to 4.6. My Dr. (Sloane Kettering)
    recommended an MRI and a lesion was found. Then, I had 4K score test. PSA up to 5.69. I'm speaking with him tomorrow and my hunch is he'll want
    me to have an MRI biopsy to get definitive idea about next step. It's been nerve wracking to say the least, but getting to the truth.

    My psa doubled and my psa percentage went down in 6 months. My doc wanted to do a biopsy. I declined. Turns out I had an infection in my prostate that a month course of antibiotics cured . So don’t let a doctor do a biopsy if it’s not needed. 🙄

      Same thing with me. Scared the hell out of me.went from 2.2 to 4.6 in a year. After antibiotics went down to 3.1 then 2.8

      An MRI should come before a biopsy. My MRI detected a lesion. Without the detection of a lesion I don't believe my Doc would have ordered biopsies, but would have ruled out other possibilities first.

    I have my PSA tested twice per year for the past 10 years. It has bumped around between 6 and 8 during this time and as high14 once. (talk about scary) My last tri-view guided biopsy (17 samples) once again showed no cancer in 2022. I'll have another one spring 2024 because of my family history of all males dying of prostate cancer. My urologist, 40 years in the biz and head of the urology department at OHSU, said I am atypical. I don't stress on PSA anymore. I also am atypical on nutrition, you might say, as well as fitness. So who knows?

    My PSA was 52 in January of 2023 with a biopsy test after , and a diagnosis of prostate cancer , I since then started a plant based no sugar diet , started working out on a rebounder every day and my PSA went down to 42 am I keeping it at bay , maybe reversing it ?

      I am glad your PSA when down my friend but 42 is still high so unlikely it was reversed. There are many medical treatment available beyond surgery or radiation so keep researching. All the best.

      My guess is as you keep it at bay it is metastasizing. In other words you are allowing the cancer to manifest in your body. You should get a PSMA PET to determine where the cancer is not just how much via PSA testing.

    Great video doctor thank you for clearing things up. I've been doing PSA tests every year ( as per Medicare) since I turned 72 and DREs every year since 65 which did not disclose any issues. My PSAs were always in 2.2-2.6 range up until 75 then my PSA jumped to 4.11. Went to an urologist who retested my PSA in 6 months and it went back down to 3.2. Again 6 months later went to 4.2. Had and MRI which indicated 2 lesions of "possible concern" and the urologist wants to do a biopsy. My question is at my age of 76 and not having any symptoms related to enlarged prostate or cancer whatsoever, is it really necessary?

    Last Friday my doctor actually called me at 8:30 at night to let me know my PSA level was 13.2 and now of course I'm freaked out and been doing tons of research. I just had a prostate MRI and see my urologist tomorrow so I'm hoping it's obviously just some sort of fluke or something

    My PSA went from 1.2 to 3.38 in a little less than 3 years. My family Doctor acted like it was nothing to be concerned about. This change in my PSA started just before I turned 71. I'm 73 now.
    These are small numbers for a man in my age group, but these small numbers jumped fast enough for me to not trust my family Doctor, and head to a Urologist. It was then that I learned what I thought to be plausible....that I had prostate cancer.
    I agree with Dr. Geo 100%. The rate of change in PSA should be followed up. It may be no big deal, or it may mean cancer. I thought it quite possibly did mean cancer, and I was right.

    First PSA 600, weeks later 900... And all of your doctors talk about numbers like four, 6,7,8,10 even 20 nobody ever talks about numbers I'm experiencing and the worst part is I feel healthy only anxiety about numbers.

      Test sucks,they should do a mri for cancer, no pain, no needle biopsy with out pain medication leaves you screaming for 20 minutes test… I think they treat prostate cancer with nothing wrong with the person?

      I'm going through the exact same thing except they say mine spread to the lymph nodes beside the prostate, I've been taking all types of supplements to support my immune system, while I'm waiting for the oncologist to take his sweet time to call me and set an appointment I'm going to start treating myself with Fenbendazol and ivermectin

      @@ebiebanzhaf4971don’t wait to be called by a doctors office. You have to advocate for yourself at every point of your recovery. You should get several opinions even when you’re already on a treatment plan. Men’s groups are a good place to hear about things you may not have considered as well.
      I have been through this process - one month left of HDT and second generation pills. It’s not easy.

      I'm Shocked... and so is my Urologist... Both of my Biopsies after the PSMA PET scan indicated uptake in both the Lymph Node and Prostate... But BOTH biopsies Pathology Reports showed 100% Benign... NO cancer! All 12 Prostate needle samples were Benign. This was good news... YET confusing news. PSMA is a VERY ACCURATE test (per the data I read)?. Your thoughts?@@DrGeoProstatePodcast

      Might be going through a reoccurence. After radiation treatment psa drop to .001. Started to trend up immediately. .046, .59,, 1.01, 3.02, 4.03. All less than 9 months. Not sure it's because of discontinuing of ADT shots, radiation failure, or it had already metastasis and the CT scan and skeleton scan didn't pick it up before my radiation treatment. What to do now?

    I was diagnosed with prostate cancer in 2017 psa was 90. Had radiation, went on Hormone drugs for a while. Had many side effects on these drugs, so stop them. I have worked out for 45yrs & now i eat a keto/carnivore diet. My psa is now 16 It has been as high as 278, so i went on hormone drugs again. Now at the age of 66yrs i have decided no more drugs, i take my chances. I feel great, fit, strong & able to do all the things that make life great. Im not worried about death.

    I am a practicing MD for over 30 yrs. I have had several patients, and one now, that have prostate cancer and their PSA was < 4. I have had several with PSA

      In fact there is no test including biopsy which says 100% cancer, they all give odds and hence each patient is different and no parameters are perfect indicators

    I normally had PSA tests around 2. A few months back it rose to 3.8. Had a free PSA a week later and it was 4.6 with free at 14. Scared me. Doc did a DRE. Normal. Put me on antibiotics and a month later it went down to 3.1. My doc said "cancer doesnt do that." Doc, do you concur? I have had a history of prostatitis in my 20s which i had major symptoms. The only symptoms this time is urgency more than normal when I needed to urinate.
    I requested an MRI but my doc said no. But he did agree to retest PSA in three months to put my mind at ease. I've known four people who died of prostate cancer and each time the docs told them "your cancers not agressive." It almost seems like PSA tests are like a magic eight ball sometimes given all the variables.

      Your PSA can go up from many things. Anything that irritates the prostate. I'm assuming that your doctor has already told you to refrain from ejaculation a few days prior to your PSA test. Doing things like riding a bike can irritate your prostate and give you an increased number too.
      I would suggest that you request a decipher test to determine if you're likely to have an aggressive form of cancer or not.
      If you don't feel as though your doctor is being aggressive enough, then find a different doctor. My PSA was only a 5 when I had my prostate removed, and the cancer had already spread. So the PSA is only one piece of the puzzle. My first urologist just wanted to keep doing PSAs every year. I found a different urologist who is much more aggressive in his search for answers. Unfortunately I waited too long to find that second doctor. So I would suggest if you're concerned that you're not getting the care you want from your existing urologist, then maybe get a second opinion.

      @@lees5761 great advise. I would add, don’t just jump into a prostatectomy. Data shows radiation therapy and HDT with a second generation drug like abiraterone is very effective if not as effective. Brachytherapy is the best according to data. However hospitals don’t focus on it because they can’t charge as much for it. I had proton radiation, but in hindsight I would look the world over to find a hospital that had, had I known this then. Good luck.

      Prostatectomy is over done. The most important thing is to determine if yours is an aggressive form or not. And there's tests for that.

      My PSA was 10-13-17 had an MRI w/wo contrast and with a prostate of 126 cc/gms was the cause. Had biopsies no cancer. Look into PAE very minimal procedural and lowers LUTS

    Had prostate cancer 2010 .
    I had my prostate removed oct 2010 .
    PSA was rising and l received 36 doses of Radiotherapy.
    Again my monitoring showed my PSA rising.
    So onto Hormone treatment , PSA almost negligible and stable.
    I was advised to take a break, but to still have my blood check every 3 months.
    3 years on my PSA has reached 9.5 .
    I am now returning to hormone treatment.
    I am 77yrs old , it’s almost now 14yrs since l started on this journey.
    I was told that l could out live this , and die of something quite different to prostate cancer.

    Isn't a MRI Prostate with & without contrast more definitive than an MRI pelvic? That's what my urologist sent me too.

    Try to follow the same regime prior to my PSA tests every 3 months or to. Gleason 6 (3+3), TURP surgery a year ago, yet my PSA still remains elevated with the most recent reading of 27, previous was 22. This seems to be contrary to what is normally seen with PSA values post TURP. PSMA-Pet scan few months ago only confirmed plenty of activity in the prostate and nothing apparently outside which is good. Had a conversation with a radiation oncologist at BC Cancer Agency and we both agree that unless the number of cores with cancer increases on my next biopsy (currently 2 test positive), no reason to get more invasive treatment. Not eligible for brachytherapy due to prior TURP.

    Is a 16 psa bad ? Dr. s haven't found a cancer with a CT scan although no biopsy has or will be done.

    I I have BPH. My total PSA has stayed between 7 and 10 since 2017. Free PSA has been at 12. I had biopsy an MRI with negative results. Values vary a point or two between testing Labs. At 76 I'm not getting anymore treatments unless PSA spikes. I test every six months.

      I take Kale powder with pomegranite juice and carrot juice PSA 4.2 .....new testing coming.
      Amazon for Kale powder or dehydrate at home.
      Take 1 tsp daily max or 1/2 that is strong enough.
      Read studies about " SULFORAPHANE = Kale Powder"

    What should I ask of my Dr. (at Mt, Sinai NY). Been monitored with cancer, for almost 10 yrs. Put off treatment. MRI-Biopsies. Low gleason score, been detected in a couple of cores ea. time, and total PSA now gone over 7. High anxiety now. 68 1/2 years old,

    I have to laugh because I listened to this on my 12 hour drive from central Idaho to Anschutz near Denver CO. I’m a year and a half post radiation and 6 months off of ADT. My first radiation treatment was in 2016, in April of 2020 (first adt vacation) my PSA went from 2.7 to 5.0. That’s when I learned they like it to take at least 11 months to double. December of 2020 is when I went off ADT so I had a PSA level that was high enough that the newly approved PSMA scan or an Axumin scan could find why my PSA was trying to double every month.
    I was a little nervous, I still have my prostate so I expect it to rise… but. I stopped Zytiga in September, I felt the Lupron wear off 2nd week of November, December PSA was still less than or equal to 0.01, January it was 0.12, February it was 0.21, and I was waiting for testing for March…. Would it almost double in a month again. I didn’t like this Dejavu and the scanxiety was sneaking in.
    I’m glad to report 0.31 from 0.21 in almost 2 months is a very welcome result.

    I had a PSA of 14 during Covid (had a vac and 2 boosters). After a year, my PSA went down to 7. Biopsy cleared me of cancer. Does vacs affect PSA? I have enlarged prostate but my urologist didn't clearly explain the initial 14 PSA.

    So I watch videos from Dr. Philip Ovadia, Dr. Ken Berry and other professionals that indicate a carnivore diet is much better than a plant based diet and they indicate the research confirms that. Dr. Ovadia is a heart surgeon that has done over 3000 heart operations. Yet my cardiologist doesn't agree and it appears you don't either. So how does one decide what life style choices are the best for overall health?

      He may have inflammation or infection. He should see a urologist. Likely too young for prostate cancer but one never knows.

    i never heard of a PSA test. My first PSA was 16.8. DRE revealed a firmness on one side of my prostate. I get my Biopsy in 23 days. Im sweating bullets...should I be? what else could it be, i have no symptoms other than occassional slow urine start. Anyone weigh in on this.

      Turns out, I have stage 2 Prostate cancer, 3+4=7 gleason, being setup with a Robotic surgeon soon. My CT and bone scans only showed the prostate cancer. Urologist says I will be ok. But Im worried about the prostate surgery cause I had a heart attack in 2016 with 1 stent. Im 67 years old.

    Is a total PSA 15.25 bad for someone who had a Urolift but is functioning okay except for getting up once at night after drinking over one gallon of water during the day?

      Maybe or maybe not. You may have a very enlarged prostate - that would be my guess, since you had a urolift performed.

      That PSA is a little high but I have seen many PSAs that high from an enlarged prostate. i would recommend a ExoDx test or SelectMdx - both urine tests and maybe a pelvic MRI to ensure the high number is related to a big prostate and not cancer. Good luck.

    Husband had a biopsy , everything came out great! Then a few months later had another psa test now it's a 7 where it was a 4 before. That's why they wanted to check before he starts taking test pellets. No one told him to not take ibuprofen and naxoproxen

      They might have missed cancer in the first biopsy of perhaps as prostatitis. I would recommend a Confirm MDx of the first biopsy to ensure it is indeed negative for prostate cancer. Good luck.

      @@johnclintonpain904 run, and have a PSMA PET done. If you have no prostate then something else is producing the PSA. Cancer could have metastasized. I hope not in your case, but I’ve never read anything else except PSA levels coming from prostate tissue or prostate cancer. All is not lost if it is the latter, but you need to get the scan.

    ED, regularly talk to 77 yearold that has cancerous prostate size if orange from mississippi.. i ask how do u urinate? he is taking some prescribed drug maybe to urinate. he is toughing it out and still going to flea mkt every Sat. maybe science could learn here.

    My PSA is currently 10.5 if you were me male blk 64 non- smoker. Exercise 3-4 days a week plus taking Ka’chava, sea moss, black seed NMN . However psa keeps growing what should I do

      You really need to visit a urologist. That is pretty high....normal PSA is four or less. Two years ago my PSA was 4.....a year and a half later it went to 6.1. I was referred to a urologist who did a prostate biopsy and it came back as cancerous. So they decided on active surveillance where they would repeat my PSA every three months for a year and then repeat the biopsy.....well after three months the PSA had jumped to 12....not what we were hoping for so they immediately ordered a MRI of the prostate and will repeat the biopsy after the results come back. The MRI allows them to accurately map the prostate and show where the tumor is so when they repeat the biopsy they will be sure to get the sample directly from the tumor for analysis. I am preparing myself mentally for having my prostate removed just in case. I am 71 years old but in good health. One other thing you probably know but black males have a much higher rate of bladder cancer but it does affect white men like me but at a lesser rate. The biopsy is not painful as you are under sedation when they do it but you will be sore for about two days and you will pee blood for several days as well. Good luck..

      @@rojoreds524 I am on the same road as you, w/ similar age and numbers. I just finished reading: You Can Beat Prostate Cancer (2nd edition) by Bob Marckini. Excellent book! He introduces and favors Proton Beam, but the book has great info about all treatments. (I am still looking at my options but at this point i do know that I will not do surgery. Good luck!)

      Go see a urologist and get an MRI. That will give you a good indication as to whether they see any tumor activity in your prostate.
      Biopsy isn't that bad. I wasn't even sedated. It was no big deal.
      Find out definitively if you have cancer or not. If you do then you'll have to make the tough decision as to which treatment is best for you.
      My PSA was only 5 when I had my prostate removed and the cancer had already spread. So I had radiation following... And unfortunately it appears as though it has returned.
      So all I can say is don't wait too long like I did.

      cancer is sneaky thing.ED here, PSA was 9, biopsy showed 3 cancerous. hellish surgery-0 PSA. 1 year PSA rising-33 rad. treatments-)PSA. 1 year PSA .02-.04 in 6 months. next PSA test JAN. 15. doctors waiting/watch for higher # to scan. may use Lupron again, but I'm feeling great except for china-covid!!!

    I heard that the inventor of the PSA test doesn’t think it’s a good indicator of prostate cancer. In his words “no better than a flip of a coin” … his position - you can have prostate cancer with a PSA of 0.7. So, should I ignore my PSA of 0.7 (66yo) and insist on an MRI ?

    You all talk about PSA test as a marker, but why not go directly to a Digital Rectal examination to get a determination if the prostate has issues.

    Had my PSA checked at 66 fall of 2021 right when i had covid. It was 15.5. I tried some natural methods and 6 weeks later it was 5. Urologist did a digital exam and my prostate was still enlarged. Fall of 2022 started having more urgency and frequency of need to urinate. Checked in Feb of 2023 and psa is 18.5😢. I get more serious about taking some natural stuff plus berberine as well as cutting back on sugars and breads. My psa drops to 5.5 my urologist drops cytoscopy idea but digital still says enlarged prostrate. I have gone from urinating up to 5 times per night to one. Dr wants another psa in 6 months. What do you think about this situation.?

      Sounds like your prostate is fine as long as you take care of yourself. Eat good and take your supplements. By the way what are you taking for your prostate?

      ED, been on cancer trip 4 years. Had all the cures, sounds like biosopy, but if you are in 70's and not worried about fast growing cancer, something else prob. get you. I'm now 67, never had symptons. except swollen prostate and bad urge to pee in cold weather! my sex life was over at 63 and i'll never forget it either!!

    My PSA was 5 or 6 most of 2 years now a week ago 14.35 something but I had fire ant bute my testicle 3 days ago before I check so I am wondering now if their venom can do something with my PSA number

    im 58 and 2 years ago when i was 56 my psa was 1.9 and now its 1.5. it went down is that a good thing?

    Yet many if not most urologists order a biopsy when total PSA is above 4. Why? Because they're up against a quota and total revenue!

      I would recommend that nobody gets a traditional biopsy anymore the PSMA is a much better tool and not intrusive nor damaging & dangerous.

    Years ago had a psa of 1.6 the next year was 2.5 doc did a retest in 6 months and was 1.8 so no biopsy
    The next year was 1.1 then a year later was 2.1 going to ask for another psa test. Digital exams are always ok with no problems. But my numbers have always gone up and down.
    I really don’t want a biopsy I’m 68. Any thing I should ask my urologist?

      biopsy not bad on me-ED- I have known other men to have swinging PSA's, but no cancer. it can go either way, whats the figure, 1 in 4 men will have this cancer by ?? 70 or so.

    I read that for every 10,000 prostates removed only one man's life is saved. Maybe the best thing is to leave all this PSA stuff alone for older men.

    Please discuss "How conclusive/accurate" the PSMA PET SCAN is. Just had mine... and uptake in Prostate was obvious.. BUT the "Odd" Uptake occurred in the Pericavial Lymph Node that my Urologist says will require a Biopsy of both the Lymph Node and Prostate before a Treatment can be determined. The Lymph Biopsy is the BIG mystery and danger because very close to my heart. I am worried.

      Ok... Update: Prostate Biopsy (12 samples) ALL benign... yet PSA has climbed to 4.0. My Urologist said to wait 2 months before another PSA and a course of Antibiotics before the PSA blood draw. Is this good advice ???

    So, my total PSA is 2.5
    My MRI reveals a prostate volume of 56 cc.
    So, 2.5 ÷ 56 = 0.04
    Did i do that right?
    So, I'm probably low risk, maybe? But, MRI also reveals 3 suspicious lesions, pi-rads 4/5😬
    Not sure what to do.
    I'm considering tackling this with a plant based diet big time, for starters. Although i eat pretty well as is. In the gym 3 times a week. In great metabolic health. I dont understand this.
    P.s.
    As a side note:
    Ive been on hormone replacement therapy, for the last 10 years. Testosterone cypionate, 120 to 200mg a week. Wonder if that had anything to do with any of this? Alot of guys in the gym are doing this, among other things, which i dont do.

      Testosterone will feed the cancer if it's there. First thing you do if you are diagnosed with prostate cancer if you opt for radiation is to be put on a Testosterone blocker. Bring the levels to near zero

    Extremely helpful. Currently undergoing hormone therapy (Orgovyx) after a couple months of tests: bone-, CT-. and PET-scans.. PSA got as high as 521(Gleason 8)(MetaPC) at one point. Now half that number PSA after just 3 weeks of therapy. Initially, it was doubling every 3-4 weeks.
    Would be interested in your opinion on DIM (d iinodolyl methane), essentially a supplement containing the same components as cruciferous vegetables. Some good research out there (happy to send) suggesting it lowers estrogen production in men. And more research indicating that estrogen is the main feeder of the cancer, not so much testosterone.
    Interesting theory that because T drops in men as they get older and Estrogen rises, prostate issues occur. Women, of course, have an E drop and T rise. Is the female reversal a contributor to breast cancer?
    Not a huge diet change for me as I've mostly been plant-based, alcohol- and drug-free and sugarless, but have ramped up cooked tomatoes, tumeric, arugula, spinach, flax oil, brussels sprouts, walnuts, pistachios, D3, calcium, and melatonin.

      There are many supplement purpoted to help with PrCa. I think curcumin is essential. I discuss supplements and formulate a few at xywellness.com. Hang in there and make sure to exercise daily, as I think that is most important. All the best!

    Hello Dr.Geo. I am 65 years old of afro/ indegenous race. I am not sure what I have. I am suffering from a pain in the urine bladder area. I use to suffer from slow urination some months ago. I use to suffer from U T I. I ttok treatment and it subsided. I sex alot. Can this cause what I am suffering from? I had P S As fone in 2013 and 2023. I was told it was normal. And is normal now..I also had a rectal exami nation. Its normal. I will do ultrasound now. I like your advice and will continue to share your information. Psa read 0.91 and 0.86.respectively. Whats your advice?

      Not sure what you mean by defined. I’m not a doctor, but I am a patient of prostate cancer. PSA is extremely accurate and very sensitive. Less than .02 on the scale. A high PSA does not mean you have cancer.
