Minecraft Button Combination Lock Using Command Blocks

Below are the 3 commands you will need to use:
setblock ~1 ~ ~ air
fill ~ ~ ~-4 ~ ~ ~-4 air
clone ~ ~1 ~-4 ~ ~1 ~-4 ~ ~ ~-4 replace
