Biggest dicepool I've rolled is a three digit pool in Exalted 2e, but in the same session there were also infinite die pools.

    I have three spectacular deaths.
First, I played a Diplomat/Ninja in a PbP Legend of Five Rings game (4e) and the first time I snuck out to do ninja stuff, I discovered that the GM was running a sandbox when two Kolat Rank 5 ninja ambushed me. I couldn't fight them of course, they did question me and I used my diplomat side to make them believe:
    - The Scorpion knew all about their operation.
    - We had an informant in their operation.
    - Their headquarters was compromised.
Then I tried to run and I got murdered. The Kolat from that point:
    - gutted and burned down their headquarters
    - sent an agent to the Scorpion ninja stronghold to grab my files... they got caught on the way out so the Scorpion knew what they were there for
    - ran a massive purge of their agents trying to find the non-existent traitor.
Second, a NSFW BESM character where my polyamorous energy channeling oni tried to interrupt the power line to the campaign final boss (ironically another player) because I rolled well and had been setting up my character to ascend throughout the entire campaign, the GM ruled the power forced me to ascend instead of killing me... it's still functionally a death because the character was not around but basically she tried to channel way more power than she was safely able to handle.
Third, recently, I collaborated with a GM for a Strahd game to set up two characters, one to die early and one to replace her. With the aim of scaring the other players and setting up a mood. I died the first session and the other players bought the hook SOOOO hard.
