Ive never seen one of them before!! Its definitely an odd ball bell box. The vents which face the wall are there to stop foam from getting to the bell box.

These boxes are so rare I only know of one in the wild and if you didn't know it, you'd think it was a light as the blue print has faded away leaving a totally opal box.
There was one of these installed on a house on the next street to us in the 1990s I remember it sounding and it had a strobe inside too
