Most institutions don't allow to prepare meds alone that is why most of the nurses choose B 😢. Letter D only emphasized LABEL check. We do check 3 times on different occasions as you mentioned but not the label, here they emphasized Label check. We get more confused. Because different institutions have different policies and procedures to follow. I just hope God will lead us simple set of questions. Thank you for helping us with our review. Hope we all pass.

Following a CVA, patients often experience changes in cognitive functions such as memory, attention, problem-solving, and emotional regulation. These cognitive changes can impact the patient's ability to manage their daily activities, including emotional responses, sleep patterns, and appetite. Assessing cognitive abilities helps determine if the observed symptoms (e.g., trouble sleeping, sadness, reduced appetite) are related to cognitive impairments that may have resulted from the stroke. Cognitive issues can contribute to emotional distress and behavioral changes, which might need specific interventions or support. B; Complete a Depression Index: A depression index is useful for identifying depressive symptoms. However, in the context of a post-CVA patient, cognitive changes can also significantly affect emotional well-being. Before addressing depression specifically, it’s important to understand if cognitive impairments are influencing the patient's symptoms. C; Strengthen Family Coping Methods: Strengthening family coping methods is valuable for long-term support, but immediate intervention should focus on understanding the patient's current symptoms and cognitive status to address the root causes of the emotional and behavioral changes. D; Screen for Pain: Screening for pain is important, but the symptoms described are more indicative of emotional or cognitive issues rather than physical pain. The primary focus should be on evaluating cognitive function, as it may be impacting the patient’s emotional state and behaviors. Assessing for changes in cognitive abilities is the most appropriate initial intervention because it directly addresses potential underlying issues that may be contributing to the patient's difficulty with sleep, mood changes, and appetite. Identifying cognitive impairments can help guide further interventions and support tailored to the patient's needs following a CVA.
Question No. 4 correct Answer is Option A. A. Risk for peripheral neurovascular dysfunction Explanation: Post-joint replacement surgery, the primary concern is ensuring adequate circulation and nerve function in the affected limb. This is because complications such as blood clots, compromised circulation, or nerve damage can lead to severe consequences like loss of limb function or tissue damage. Assessing and monitoring neurovascular status (checking for pulses, sensation, and movement) is crucial in the early post-operative period to detect any signs of neurovascular compromise.
