Please note, answering anything besides she is dead at the end will void the trophy/achievement. Outside of that, just follow the guide and you should be fine.
Just want to point out, you can kill any of those repair bots you see around Central Production B to get the leg. I completed this quest twice by going all the way to the one robot in the video, but another video shows that you can kill the robot located just outside the entrance to R&D in Central Production B at the bottom of the staircase where the Vending Machine gang hangs out 👍

If you choose the "Maybe..." option the quest will bug out: Alec will advance his dialogue as if you accepted the quest, but the internal quest checker in the game does not count it as if you did - and as a result no leg will never drop and you fucked the quest. :(
To make it worse, I am on NG+. In NG he spazzed out and just ran out of Ops (leaving poor Maddy all alone) and got greeted by the shocksticks of Security mooks who promptly wasted him. >_>
Well, you can always lay the smack down on him in NG++ and then do it in NG+++. lol. Joking aside, that really sucks. You should make copies of your saves. That is what I do if I am worried about something.
I hated this quest, same with Jo you make the wrong choice and bam no fucking achievement. Never tell that guy that Maddy could still be out there after you give him her necklace, his dialog bugs out or something and just repeats the same line over and over again when talking to him and you will not receive the achievement. With Jo never "steal" the Equalizer weapon from her hidden room(the one with the audio log in it that is through a corridor filled with toxic gas), it doesn't prompt giving her the 15000 scrap and even if you give her the weapon back you still don't get the achievement and she goes and kills Hobbes anyway.
anyone else hate the music in the safe areas as much as me? it's almost like they did it on purpose as a joke, I can't imagine anyone would think it's a good idea to put that crappy song on nonstop repeat the entire game.
you can change it with the patch (on beta branch on steam). In My Documents/the surge/songs (create it if it's not there) and put a FLAC, OGG or WAV files it will play them instead.

There are two versions of every boss weapon, besides the last boss which has two different weapons. In the case of Firebug's, it has fire, it does elemental damage and has flame effects on certain attacks.

Well fuck me, I saw robot, charged it and unleashed my staff combo only to notice name Maddy a secold later. But it was too late, I wrecked her. :D

You have to break off all of Firebug's legs. The video I uploaded of me fighting him shows it, if you want to see how it is done.

Yeah. It requires (if I recall correctly) the overcharge by the two crystal enemies and then you take the lift to the right of the door.

She says "..." if I recall correctly, which I believe is just there to give you an idea of her location.
