He’s caused her too much pain and he’s in pain for having put her in this position in the first place (in effort to protect her he pushed her towards Lex). This scene was very well acted and made fans ache for more moments even though it was tough to watch.

He told Chloe after Lana trapped Chloe in the wine cellar that giving up Lana was the hardest thing he ever had to do and that if she didn’t know that she didn’t know him very well. ❤️❤️
It's nice that Clark doesn't always use his powers but he could've used his vision to see if Lex was in the house, PS if Lana was ever pregnant, why couldn't he hear the fetus's tiny heartbeat and then x-ray her body to see it?
Lana you are being mean Clark is just trying help you and you are sad about nothing because you are not really pregnant. And you will soon learns about that and then you really be crying and hate lex for it and then you can go crawling back to Clark like you always do
She hated Lex not for faking her pregnancy but for all his crimes. After finding out the truth about their ‘baby’, Lana just became more suspicious. The following episode, she was able to get the case in his car and learned his crimes. Only after that Lana hated Lex. At the end of season 6 when she was gonna leave him she mentioned about her fake pregnancy not his crimes towards other people. That’s because Lana wanted to humiliate him more as if _’You’re such a monster that you’re merciless even to your beloved ones, not only strangers’._
She wasn't mean she was in pain and set boundaries. Did anyone ever hurt you so badly that you can't even stand to be in their presence anymore ? That's what happenned in this scene. Clark caused Lana a lot of pain when he told her "I don't love you' and broke up with her. So him coming here trying to comfort her after everything that happened between them was too much for her to bear.

Because he has to focus to be able to superhear or X-ray. If he doesn’t think about it he won’t hear it.
