So, I believe that, in the end, you sort of _become_ Dark Six, which is why there's no Dark Six appearance, unless it's just too dark to see it.

Another two reasons to believe the geisha might be six's mother: When six's face can be seen (in this particular video, the shot with her, the tv and the janitor) it looks completely white and her eyes look fully black, like the geisha's mask (perhaps neither of them are wearing a mask and this is simply their demonic face) On top of that, maybe the idea of six eating her mother, gaining her powers and killing everyone shows that you have been playing as the villain all along, and she grows up to be just like her mother- monstrous, eating live creatures as opposed to already available food because she prefers it.

In reference to the Janitor looking at a room where it seems a child could be sleeping in a bed with a shadowy figure looming in, here's my theory--every creature and monster you see in the game was once a child. Six is clearly of significance, but perhaps the others were abducted from random places, the whole "monster under the bed" or "in the closet" scenario. Also, Six woke up in the Maw, she wasn't immediately caged in any way that we could tell. Most, if not all, of the other children are still in some type of containment. It's just something to ponder.

i think the hat belongs to Indiana Jones because you see it after a really close call and Jones after every close call he has , he saves his hat the last moment (he also wears a same looking hat)

There is a way to see Six's face. If you light a lamp and stand in front of it and then turn on your lighter and look up You can kinda see it.
I think the red butler was originally going to be the Maw's bellboy. He wears a suit similar to that of a bellboy. Then again, that's just a theory...A GAME THEORY! (Don't sue me MatPat)
